Description

Abstract
摘要:根據世界衛生組織統計,目前全球有四億多人口患有糖尿病,每年並造成一百六十萬人死亡,是目前最重要的全球健康議題之一。糖尿病症可由個人血糖的監控來加以進行治療與控制病情,近年來,糖化血色素(HbA1c)已被世界衛生組織以及美國糖尿病協會共同提出作為糖尿病檢測指標,其可提供患者二到三個月的長期平均血糖狀況,因此檢測糖化血色素可以減少病患遭受一般使用採血針與血糖儀等進行侵入式檢測的疼痛,並且降低感染風險。
傳統分析糖化血色素的方法有陽離子交換管柱、高效能液相層析法、以及親和力管柱分析法等,然而這些檢測設備昂貴、樣品製備過程複雜且耗時、及需要專業人員操作等因素,限制該系統可直接由終端使用者使用於個人疾病管理上。目前既有之可攜式糖化血色素檢測產品可提供高靈敏度的檢測能力,然而檢測設備價格以及單次檢測費用高,因此無法達到高普及化之個人健康照護需求。
為了克服上述挑戰,本計畫吾人將開發一項創新整合型微流體式快速糖化血色素檢測系統,研究內容將進行下列關鍵技術與功能探索,(一)積層製造液體驅動幫浦,(二)創新式試紙流體控制工具開發,(三)慣性流血液樣品簡易前處理,以及(四)整體系統整合與優化,使研究成果達到檢測程序簡單化,並且進一步降低整體檢測單價之目的。
Abstract: According to the report from the World Health Organization (WHO), there were an estimated 420 million people having diabetes in the world, and approximately 1.6 million people died every year. Diabetes is a metabolic disease characterized by elevated levels of blood glucose (hyperglycemia). Raised blood glucose leads over time to serious damage to eyes, blood vessels, kidneys, nerves and increases the risk of heart attack and stroke. Recently, Hemoglobin A1c (HbA1c) has been recommended by WHO and ADA as an indicator for early detection and screening of diabetes mellitus. Unlike daily blood glucose measurement using finger prick and glucometer, HbA1c gives information of average blood glucose over the previous 2-3 months. In addition, the diabetes monitoring via the level of HbA1c can be less painful and lower the infectious rick.
The traditional HbA1c detection techniques such as ion-exchange chromatography, high-performance liquid chromatography, and boronate-affinity chromatography require expensive equipment, complex sample preparation, long turnaround time, and well-trained technician, so they are incompatible with on-site diabetes monitoring. There are some commercial portable HbA1c test devices have been developed and available on the market with satisfactory detection ability. However, the equipment and the test cost still limit the availability for the huge number of end users.
To overcome these challenges, here I propose a unique microfluidic system that provide the following critical techniques and functions including (a) 3D printed portable manual fluidic pump, (b) fluidic manipulation on the paper devices, (c) inertial fluidic for sample pretreatment, and (d) diagnostic system integration and optimization, for rapid, simple and cost-effective diabetes monitoring.
