標題: | 行政院國家科學委員會專題研究計畫成果報告:影響大學醫學生專科選擇之因素探討 | 作者: | 呂碧鴻 | 關鍵字: | 醫師生涯發展;生涯選擇;醫學教育;physician career development;career choice;medical education | 公開日期: | 1999 | 出版社: | 臺北市:國立臺灣大學醫學院社會醫學科 | 摘要: | 選擇醫學為專業者一向比其他學科的 學生在決定其生涯職志的時間要早一些, 但是醫學生仍然必須面對專科和次專科領 域的選擇問題,以及決定將實施醫療行為 的醫院類型等問題,改變生涯計劃的醫學 生並不少見,而專科的選擇是否能夠配合 自己的興趣與個性,對於往後的醫師生涯 有著相當重大的影響。因此本研究的主要 目的在瞭解醫學生的生涯發展狀況,以及 探究對醫學生生涯發展的相關影響因素。 本研究是以台大醫學院醫學系四到七 年級的學生為主要對象,以問卷調查的方 式搜集資料。研究結果發現在傳統的大 科,如內科、外科和小兒科等,仍然是同 學們最有興趣的科別,其次則為家醫科與 精神科。從年級來看雖然略有一些差異, 但大至上仍以這幾科較受同學們的喜愛。 而影響大多數同學們對這些科別的選擇, 是以自己的興趣或是配合自己的個性為 主。 此外,在探討未來行醫地點的偏好 時,發現大多數的同學仍然希望能留在大 台北地區,其次才是自己原來居住的地 區。而向來缺乏醫療人員的離島地區,同 學們則興趣缺缺。 最後有關醫師生涯發展量表的測量結 果顯示,可以發現基本上醫學生在發展其 職業的生涯時,是呈階段性的漸進式發 展,但是有幾個例外,其一是在確認以醫 師為職業生涯的發展,超過應該在前一個 階段便已澄清各種可能的生涯選擇。另 外,專科澄清階段和下一個專科確認階段 也是類似的,而各個年級都有相同的情 形。 本研究建議未來能有更多相關的研究 投入,作更廣泛和深入的研究,特別是連 續性的多年期追蹤研究之迫切需要,以瞭 解專業生涯發展的動態過程。 Those who choose medicine as career generally make their decisions and have their professional identity earlier than the students from other specialty. However, they still confront the problem that they must choose from among many specialty and subspecialty alternatives, types of medical practice, and practice of location. According to the results of previous research, the percentage of medical students who change their career plan during medical school is pretty high. Therefore, the purpose of this study is to understand medical students’ career development thoroughly and investigate the determinant and influence of related factors on career development of medical students. By surveying 536 medical students in medical college at National Taiwan University, the results of this study found the most of the medical students were interesting in selecting internal medicine, surgery, pediatrics as career. Students’ interests and personality were the major determinants. In additions, the results of preference of practicing location showed that the most of the students preferred to staying in Taipei area, and their home towns were their second choice. No student wanted to leave island for their practicing. To measure career development during the exploration and early establishment stages of a physician’s career, it is important to construct an inventory. The results of this study indicated that the career development in medical students were progressively developed stages by stages. However, the stages of crystallizing a career preference and specifying physician as career preference, and the stages of crystallizing a specialty preference and specifying a specialty preferences were somewhat in the reverse order. This study also suggested that more related research were needed in the future. Especially, the longitudinal studies of medical students’ career development were highly recommended. |
