Publication All-cause mortality in patients with type 2 diabetes in association with achieved hemoglobin A1c, systolic blood pressure, and low-density lipoprotein cholesterol levels(Public Library of Science, 2014) ;Chiang H.-H.; ; ; ;Chen Y.-C. ;See T.-T. ;Chen H.-F.Chiang H.-H.;Fen-Yu Tseng;Wang C.-Y.;Chen C.-L.;Chen Y.-C.;See T.-T.;Chen H.-F.Methods: A retrospective cohort of 12,643 type 2 diabetic patients (aged ?18 years) were generated from 2002 to 2010, in Far-Eastern Memorial Hospital, New Taipei city, Taiwan. Patients were identified to include any outpatient diabetes diagnosis (ICD-9: 250), and drug prescriptions that included any oral hypoglycemic agents or insulin prescribed during the 6 months following their first outpatient visit for diabetes. HbA1c, SBP, and LDL-C levels were assessed by the mean value of all available data, from index date to death or censor date. Deaths were ascertained by matching patient records with the Taiwan National Register of Deaths.Results: Our results showed general U-shaped associations, where the lowest hazard ratios occurred at HbA1c 7.0-8.0%, SBP 130-140 mmHg, and LDL-C 100-130 mg/dL. The risk of mortality gradually increases if the patient's mean HbA1c, SBP, or LDL-C during the follow-up period was higher or lower than these ranges. In comparison to the whole population, the adjusted hazard ratio (95% CI) for patients with HbA1c 7.0-8.0%, SBP 130-140 mmHg, and LDL-C 100-130 mg/dL were 0.69 (0.62-0.77), 0.80 (0.72-0.90), and 0.68 (0.61-0.75), respectively.Conclusions: In our type 2 diabetic cohort, the patients with HbA1c 7.0-8.0%, SBP 130-140 mmHg, or LDL-C 100-130 mg/dL had the lowest all-cause mortality. Publication Human herpes virus reactivations and dynamic cytokine profiles in patients with cutaneous adverse drug reactions - A prospective comparative study(Blackwell Publishing Ltd, 2015) ;Chen Y.-C. ;Chiang H.-H.; ;Chang C.-Y. ;Chen K.-L. ;Yang C.-W. ;Lee Y.-H.; Chen Y.-C.;Chiang H.-H.;Cho Y.-T.;Chang C.-Y.;Chen K.-L.;Yang C.-W.;Lee Y.-H.;Chia-Yu ChuBackground Sequential human herpes virus (HHV) reactivation is well known in drug reaction with eosinophilia and systemic symptom (DRESS), but such a phenomenon has seldom studied in other types of cutaneous adverse drug reactions (cADRs). Moreover, the association between viral reactivations and cytokine or chemokine changes is largely unknown. We aimed to evaluate the viral reactivation rates of HHV-6, HHV-7, Epstein-Barr virus (EBV), and cytomegalovirus (CMV) in different cADRs and their impacts on clinical prognosis. Cytokine and chemokine changes with viral reactivations were also examined. Methods A prospective study was conducted to monitor the viral statuses of patients with different cADRs by polymerase chain reaction and serum-specific antibody titers. Changes in plasma cytokine and chemokine levels were also evaluated by sequential blood samples. Results Among the various cADRs, HHV-6 reactivation was only observed in DRESS, but EBV and CMV could be detected in other cADRs. Many proinflammatory cytokines and chemokines, including interleukin (IL)-1β, IL-2, IL-6, interferon-γ, tumor necrosis factor-α, were significantly lower in DRESS patients with HHV-6 reactivation when compared to those without HHV-6 reactivation. In addition, these mediators were significantly lower before and during HHV-6 reactivation, compared to cytokine levels after HHV-6 reactivation in the same patient. Conclusion HHV-6 reactivation was only observed in DRESS patients, not in any other cADR. In DRESS patients, some proinflammatory cytokines were significantly lower before or during HHV-6 reactivation. ? 2015 John Wiley & Sons A/S. Publication Improvement on Physical Properties of Pullulan Films by Novel Cross-Linking Strategy(2017) ;Chen C.-T. ;Chen K.-I. ;Chiang H.-H. ;Chen Y.-K.; Cheng K.-C.;Chen Y.-K.;Chiang H.-H.;Chen K.-I.;Chen C.-T.Pullulan based films possess several advantages, including high transparency, low toxicity, good biodegradability, good mechanical properties, and low oxygen permeability, are preferable for food packaging. The application of pullulan films on food packaging, however, has inherent disadvantage of high water solubility. In this study, glutaraldehyde and glycerol were used as the cross-linking reagent and the plasticizer respectively to improve water resistance and physical properties of the pullulan films. Effects of cross-linking degree on physical properties, including water absorptions, swelling behaviors, water vapor permeability and tensile strengths of films were evaluated. FTIR results demonstrated that the pullulan films were successfully cross-linked by glutaraldehyde. The tensile strength of pullulan films could be enhanced significantly (P < 0.05) when glutaraldehyde was between 1% and 5% (w/w); nevertheless, the amount of glutaraldehyde above 20% (w/w) led to films brittleness. Publication Soymilk isoflavone conversion prediction by adaptive neuro-fuzzy inference system(2015) ;Chiang H.-H. ;Chen K.-I. ;Liu C.-T. ;Hsieh S.-C.; ; Cheng K.-C.;Hsieh S.-C.;Liu C.-T.;Chen K.-I.;Chiang H.-H.In this study, a model for prediction of soymilk isoflavone glycoside conversion was constructed using adaptive neuro-fuzzy inference system (ANFIS) techniques. We chose aeration rate, cultivation duration, and the amount of isoflavone glycoside as the three inputs and the yield of isoflavone aglycone as the single output to develop the prediction model. The average root mean square error (RMSE) of the output over 50 training epochs for genistin and daidzin conversion processes were 3.43 ¡Ñ 10-5 and 4.59 ¡Ñ 10-5, respectively, which demonstrates that the established models were significantly well-trained. The values of RMSE and MAE for genistin and daidzin conversion processes were (0.46, 0.83) and (0.36, 0.63) during testing, which suggests that the yield values predicted by the ANFIS model closely matched the actual values. The results implied that ANFIS is a powerful tool for predicting isoflavone conversion during fermentation processes. This study aimed to investigate the pathology and clinical significance of sudomotor denervation. Methods Skin biopsies were performed on the distal leg of FAP patients with a follow-up duration of 3.8 ± 1.6 years. Sudomotor innervation was stained with 2 markers: protein gene product 9.5 (PGP 9.5), a general neuronal marker, and vasoactive intestinal peptide (VIP), a sudomotor nerve functional marker, followed by quantitation according to sweat gland innervation index (SGII) for PGP 9.5 (SGIIPGP 9.5) and VIP (SGIIVIP). Results There were 28 patients (25 men) with Ala97Ser transthyretin and late onset (59.9 ± 6.0 years) disabling neuropathy. Autonomic symptoms were present in 22 patients (78.6%) at the time of skin biopsy. The SGIIPGP 9.5 and SGIIVIP of FAP patients were significantly lower than those of age- and gender-matched controls. The reduction of SGIIVIP was more severe than that of SGIIPGP 9.5 (p = 0.002). Patients with orthostatic hypotension or absent sympathetic skin response at palms were associated with lower SGIIPGP 9.5 (p = 0.019 and 0.002, respectively). SGIIPGP 9.5 was negatively correlated with the disability grade at the time of skin biopsy (p = 0.004), and was positively correlated with the interval from the time of skin biopsy to the time of wheelchair usage (p = 0.029). Interpretation This study documented the pathological evidence of sudomotor denervation in FAP.
