Primary antiphospholipid-antibody syndrome with pituitary apoplexy and adrenal insufficiency: A case report

Abstract
A 54-year-old man with end-stage renal failure, on continuous ambulatory peritoneal dialysis, developed bilateral adrenal hemorrhagic infarction with adrenal failure after hernia surgery and pituitary apoplexy and a minor stroke after nephrectomy. Anticardiolipin antibodies were detected in high titer and persisted for more than 7 months. He remains well without further thrombosis and is taking steroid replacement, warfarin and low dose aspirin anticoagulation. In patients with endocrine gland apoplexy, either with or without clinical symptoms of hormonal deficiency, antiphospholipid-antibody syndrome should be considered by obtaining appropriate image and coagulation studies that confirm or refute this diagnosis.
