Effect of Post Operative Infusion of Amino Acid and Total Parenteral Nutrition on Muscle High Energy Phosphates

Abstract
This study examines the effect of hypocaloric nutritional regimen and of total parenteral nutrition on muscle high- energy phosphates in patients following severe post- operative complications. Six severely catabolic patients were assigned to receive mean 8 kcal/kg/day of special amino acids for 4 days and then shifted to total parenteral nutrition (TPN), mean daily calories were 42 kcal/kg/day for 1 week. A percutaneous muscle biopsy was performed before and after each nutritional regimen. Samples were analyzed for adenosine triphosphate (ATP), adenosine diphosphate (ADP ), adenosine monophosphate (AMP), phosphocreatine (PC), free creatine (CR ). At the end of amino acid infusion only, the ATP, ADP regimen, there was no significant change in the content of high-energy phosphates. These results suggest that a hypocaloric amino acid infusion of different composition from a previous study induces the same decrease in highenergy phosphate levels in skeletal musle. The cause of this decrease may be an impaired ability to utilize the substrates and thus increase the energy needed by the cells. On the contrary, the TPN with a high amount of calories are able to maintain the cellular energy levels.
