Change in LDL-C and Use of Lipid-Lowering Agents Among Coronary Artery Disease or Myocardial Infarction Patients: A Retrospective Cohort Study

Abstract
Background/Purpose: Achieving optimal levels of low-density lipoprotein cholesterol (LDL-C) is paramount for those with coronary artery disease/ myocardi al i nf arcti on ( CAD/MI ) due to i ts di rect i mpact on cl i ni cal outcomes. For older adults, who often face additional health challenges and decreased resilience, managing LDL-C becomes even more crucial in safeguarding against the recurrence of life-threatening cardiovascular events. We ai med to eval uate the change i n LDL-C l evel s and l i pi d-lowering agents (LLAs) use within one month after a CAD/MI event in a medical center in Taiwan. Methods: In this retrospective cohort study, we included patients who were admitted for a documented CAD/MI, underwent coronary angiography (index event), and received lipid testing at the index event and at 6-month follow-up between January 1, 2012, and June 30, 2016 using the integrated database of National Taiwan University Hospital (NTUH-iMD). Use of LLAs within 1 month after the index event was evaluated. An LDL-C of < 70 mg/ dL at follow-up was used as the goal attainment per the 2011 ESC/EAS guideline. Results: Of 881 patients identified (median age: 64 years (Q1–Q3, 56.0– 72.0)), 717 (81.4%) were male. One-fifth of patients (19.0%) with baseline LDL-C ≥ 130 mg/dL did not receive LLA after the index event. At the 6-month follow-up, 255 patients (28.9%) attained the goal of LDL-C control: the proportion of goal attainment ranged from 16.4% to 57.1% by LDL-C groups at the index event. Conclusion: Despite the endorsement of LDL-C control in clinical guidelines, this real-world study highlights a concerning gap in achieving optimal LDL-C goals among adults with CAD/MI in Taiwan.
