Abstract
本個案研究旨在探討一位産婦在産下高危險性新生兒後的調適過程。本 研究中研究者以研究者即參與者方式進行資料收集,將護理個案期間與個案互動之行 爲過程加以記錄,再以內容分析法予之整理歸納。由結果中發現個案的行反應分爲二階段,即(一)震驚混亂期,(二)調整重建期。在震驚混 亂期,個案之行爲方式有(1)負向情緒反應,(2)對自己爲確保胎兒安全 所作的努力感到挫折,(3)對新生兒狀況的不確定。在調整重建期,個案所表現出來 的行爲包括:(1)對新生兒身體狀況再比較,(2)認識新生兒醫療狀況,(3)認 識新生兒的身體特性,(4)學習照顧新生兒的工作。因此在護理個案期間,研究者依 個案的反應及需求提供適當之照護,以協助個案調整自我期望與接納新生兒並負起母 親的角色工作。 This case study explores the adaptive process of a mother with a high risk newborn baby. The researcher as a participant engaged in nursing care and data collection. During nursing care process data were interpreted by the content analysis method. The results showed that the behavior reaction of the mother was divided into two stages: 1. shock and chaotic stage; 2. adjustment and reconstruction stage. During the shock and chaotic stage , the behavior modes of the mother included: (1) negative emotional response; (2) remembering the effort for ensuring the safety of the fetus; (3) uncertainty about the status of the newborn baby. During the adjustment and reconstruction stage, the behaviors demonstrated by the mother included: (1 ) recomparison of the physical condition of the newborn baby ; (2) recognition of the medical treatment condition of the newborn baby; (3) recognition of the physical characteristics of the newborn baby; (4) learning the work of taking care of the newborn baby. The researcher provided the proper nursing management to match the case’s physical and psychosocial demands, in order to assist the case to adjust to and take responsibility for the mother role.
