Development of a Novel Measure, the Trauma-10, for Assessing the Quality of Life in Patients with Rib Fractures

Abstract
IntroductionStudies on the assessment of the quality of life of patients with trauma are scarce. This study aimed to develop a trauma-specific measure to provide reliable and valid quality-of-life information in a cohort of patients with rib fractures.Materials and MethodsData on quality of life were collected using the 36-Item Short Form Survey (SF-36) with a follow-up period of 6 months. The 12-Item Short Form Survey (SF-12) scores were extracted from the SF-36. Generalized estimating equations were used to evaluate the longitudinal change in each item. Factor analysis was performed to assess the factor structure.ResultsSeventy-one patients with rib fractures were enrolled. The Physical Functioning, Role Physical, Bodily Pain, and Role Emotional domains showed significant changes over time; therefore, 10 related items were selected for our originally generated measure, the Trauma-10. The SF-12 and the Trauma-10 showed excellent internal consistency and test–retest reliability. A four-primary factor and one-secondary factor structure represented the SF-12, and the goodness of fit was unsatisfactory (Chi-square statistic = 544.5612, df = 49, p < 0.0001, Root Mean Square Error of Approximation [RMSEA] = 0.2216, and Comparative Fit Index [CFI] = 0.7632). The Trauma-10 model loaded on three factors, Physical Ability, Functional Limitation, and Pain and Emotion, and showed excellent goodness of fit (Chi-square statistic = 40.8349, df = 29, p = 0.0712, RMSEA = 0.04451, and CFI = 0.9905).ConclusionThe newly developed trauma-specific instrument, the Trauma-10, was able to assess long-term outcomes and increase the validity of the measurement of quality of life in patients with trauma.
