Abstract
With the recent advent of the adjuvant chemotherapy, image study techniques and the modern surgical skills, the limb salvage procedures become more and more common for the treatment of malignant bone tumors in the recent two decades. The survival outcomes and the local control of the tumors by limb salvage procedures have been proven to be as good as amputation. Because of wide resection of the tumor will cause extensive loss of bone and soft tissue after limb salvage operation, various kinds of joint and/or bone reconstruction techniques have been developed to treat with malignant bone tumors in the different locations. The usual reconstruction methods for the periarticular bone tumors include the arthrodesis, resectional arthroplasty, and total/partial joint replacement. These procedures can be achieved by using autograft reconstruction, allograft reconstruction, micro-surgical reconstruction, allograft/prosthesis composite, and custom-made prosthesis, etc. Since knee in one of the most common location for the primary malignant bone tumors, especially the osteosarcoma in the adolescent patient population, the custom-made knee prosthesis has been well developed for the reconstruction since several decades ago. The early single-axis fixed-hinged knee prosthesis used for reconstruction after wide resection of the malignant bone tumor in the distal femur or proximal tibia can provide a good initial result. However, many complications occurred a few years later, especially the excessive wear, infection, and loosening of the prosthesis caused by the increased torsional stress. Therefore, various kinds of rotational hinged knee prosthesis have been designed and improved the functional outcomes and the survivorship of these prostheses. The modular system of knee prosthesis with a rotation hinge has been widely accepted in the recent two decades. We will discuss the role of the rotation hinge of these knee prostheses in this article.
