Surgical Management of Sternal Osteo-Chondritis after Median Sternotomy

Abstract
There were 20 cases of sternal infection after open- heart surgery from Dec. '87 through jan. '89 at National Taiwan University Hospital. 14 cases out of them developed chronic sternal osteochondritis. One of them died of profund sepsis and multiple organ-failures, and another one refused any further operation with a persisted discharging sinus. The other 12 were treated with aggressive debridement, continuous Betadine irrigation of pectoris major turn-over flap tampon according to the extent and chronicity of the infections. Ten cases have good results, the other two have recurrence due to the incomplete resection of the infected cartilage. Knocking tenderness is the most correct indicator of chondritis involvment. The complete debridement and erradication of dead space are the principles of treatment. The turn-over muscle flap tampon is only used in those cases with sternal dead space after debridement, otherwise a Betadine close irrigation is the better choice while the chest cage is still pliable.#C2291002
