Intraaortic balloon pumping for myocardial failure in cardiac surgical patients

Abstract
From July 1976 to July 1991, 92 cardiac surgical patients received intraaortic balloon pumping (IABP) when they did not respond to high dose catecholamine. They included 53 patients with postoperative cardiogenic shock, 24 patients with inability to be weaned from cardiopulmonary bypass, 11 patients with preoperative myocardial infarction and cardiogenic shock, 4 patients with postoperative ventricular tachycardia. The mortality rate of the 4 groups was 53%, 42%, 73% and 50% respectively. The mortality rate for females was higher than that for males (73% vs 41%). Among the various diseases, the best survival rate (54%) was noted in the patients with coronary artery disease. Leg ischemia was suffered in 23% of patients, among whom 10 patients were relieved of ischemia by only removal of the balloon catheter; another 8 patients required emergency thrombectomy, and the other 3 patients received femorofemoral bypass grafting. Rupture of the balloon as evidenced by blood in the catheter was noted in 5 patients (5.4%), for whom immediate removal was required. False aneurysm of the femoral artery at puncture site was noted in 2 patients (2.2%), who received surgical repair uneventfully. Because of an improved survival rate compared with medical treatment only and an acceptably low complication rate, it was concluded that for postoperative low cardiac output syndrome, early application of IABP is recommended.
