Abstract
Objective Omphalopagus conjoined twins with shared cloacal exstrophy is a rare and easily missed diagnosis in early pregnancy. We emphasize the importance of early prenatal diagnosis and management to prevent complications.Case reportThis is a 32-year-old gravida-3, para-0 woman referred at 19th week of gestation age for conjoined twins diagnosed as omphalopagus. Initial evaluations misidentified the condition due to the presence of a large cyst, initially thought to be simple amniotic fluid. Advanced magnetic resonance imaging confirmed shared bowel structures and a significantly dilated cyst, suspected to represent a shared exstrophied bladder.ConclusionThis case highlights the rarity and complexity of conjoined twins with cloacal exstrophy and underscores the importance of meticulous prenatal evaluation for early detection of associated anomalies. Advanced imaging and prenatal intervention play a pivotal role in optimizing maternal and fetal outcomes.
