The Effectiveness of Utiliaing Case Manager for Discharge Planning at a Medical Center

Abstract
Aims: To identify the case management tactics in discharge planning for overstay patients, and the effect they have on the average length of stay, length of stay for overstay patients, successful rate of referral to long -term care facilities and average cost per hospitalization. Method: A case manager monitored patient care plans, attended to patient discharge planning, assessed patient satisfaction regarding our discharge planning services, received the overstay patients, organizing the follow up evaluation outcomes, filed data, completed statistics and analysis of data (including the average length of stay, length of stay for overstay patients, unplanned readmission rate, successful rate of referral to long- term care facilities and patient satisfaction. Results: The total number of patients served was 442, and 6, 685, in 2000, and 2001 respectively. Our data showed a decrease of 0.9 days in the average length of stay, ( from 11.5 to 10.6 days), a decrease of 0.8% in length of hospital overstay , (from 8.8% to 8.0%), a decrease of 1.2% in unplanned readmission rate,( from 3.1% to 1.9%), while rates of successful transfer to long-term care facility remained 3.6%. We achieved our discharge planning goal with shorten length of stay, and 90% positive patient satisfaction
