Abstract
在現今工業化的時代中，甲醇(methanol, methyl alcohol)的使用率非常高，因此我們必須對於甲醇中毒的影響多加了解。儘管在台灣地區，甲醇中毒雖非最常見，但其引起的併發症，如嚴重的代謝性酸中毒，眼睛病變，及其他如腎衰竭等症狀，需予以即時治療。甲醇中毒的早期症狀不明顯，但如能即時給予解毒劑或血液透析治療，也許就會有較良好的預後。本篇報告是就一件罕見的飲用含甲醇的酒而造成中毒的病例所進行的研究，該名46歲女性病患雖無嚴重的全身性症狀，卻造成視神經萎縮(optic atrophy)。基於防患未然，本篇涵蓋甲醇中毒的代謝過程、臨床表現、診斷方式、治療和預後。再者，因甲醇中毒的不良預後，希藉此呼籲大家重視對甲醇中毒之即時診斷及治療的重要性。 Methyl alcohol is a very common chemical used in industrialized society. The medical profession needs to be very concerned about the toxic effects of methanol alcohol. Although methanol poisoning is not very common in Taiwan, serious complications, such as metabolic acidosis, ocular disorder and renal failure require prompt treatment when they occur. The early signs of toxicity of methanol are not apparent. However, in most cases, prognosis depends on early and prompt treatment of the methanol antidote or hemodialysis. This paper centers on a rare case of a patient who drank methanol alcohol causing toxicity. The 46-year old female patient had serious optic atrophy, despite equivocal systemic symptoms. Based on this study, this paper concludes by offering suggestions for preventive measures and demonstrates the clinical symptoms, signs and prognosis of the methanol toxicity. Furthermore, we emphasize the importance of prompt diagnose and therapy of methanol intoxication because of its poor prognosis.
