Effect of Different Health Teaching Approach on Breast-Feeding Mothers' Worries and Feeding Method One Month after Delivery

Abstract
本研究設計為準實驗法。研究對象為344名於某教學醫院生産,且産後 初期哺餵嬰兒母乳的產歸。依產後初期住院期間所接受之不同衛教區分為實驗組Ⅰ及 實驗組Ⅱ。兩組差別在於實驗組Ⅰ有關哺母乳可能發生之問題及處理方法係依個案發 生問題之狀況隨時提供衛教。實驗組Ⅱ則有系統的預先給與衛教。本研究目的的則在 按說此兩組産歸在産後一週及滿月時所感受的困擾及其滿月時哺乳的方 式有無差異。研究結果發現産後一週時,接受不同衛教的兩組産歸對哺 母乳所感受之困擾並無差異。但滿月時,實驗組Ⅰ在「擔心造成母嬰身體及社會功能 的不良影響」、「擔心無法順利哺餵新生兒母乳」、及「擔心新生兒無法得到質佳量 足的母奶」三方面之困擾均較實驗組Ⅱ爲低(p<0.001)。但在滿月時的哺乳方 式方面,兩組個案則無顯著差異。本研究也發現,哺線乳歸所感受之困擾 與其滿月採用的哺乳方式有關。 This research was designed as a quasi- experimental study. Three hundred and forty-four breast- feeding puerperas were recruited from a teaching hospital. The subjects were divided into experimental group I and group II. In group I, teaching about the problems and management of breast-feeding was provided in accordance to the subjects’ condition. In group II, the systemic health teaching was served before the related issues occured. This study intended to understand how the different health teaching methods were related to the subjects’ emotional disturbance within one week or at one month after delivery, and their feeding methods one month after delivery. The results showed: (1) there was no difference between the two groups in the subjects’emotional disturbance one week after delivery; (2) the experimental group I had less concern about three factors:“whether breast-feeding may cause disadvantage of mother and baby”, “whether she can feed her baby smoothly”, and“whether the baby can get good quality and quantity of human milk”. We also found that (3) the health teaching content was not related to the puerperas’ feeding methods one month after delivery and (4 ) the subjects’ emotional disturbance was significantly related to their one-month- after-delivery feeding methods.
