Abstract
本研究是以免疫組織化學消研究149例原發胃癌的c-crbB-2致基因蛋白表現。其中38 例(25.5%)的腫瘤具有可代表基因擴大表現基因擴大表現的陽性染色。在分析各種 臨床病理因素後,c-erbB-2蛋白表現僅在組織型態具有統計學上的差異〔分別為:39 .3%於腸型胃癌,15. 4%於廣泛型,p<0.01〕而與年齡,性別,腫瘤部位,肉眼部位 ,肉眼型態,大小,侵犯深度,淋巴結轉移,遠處轉移,及期別無關。但是在10例轉 移的淋巴結中,有7例具有c-erbB-2蛋白表現陽性染色,包括3例原發腫瘤為陰性染色 的病例。這顯示c-erbB-2蛋白表現與胃癌的轉移可能有關。c-erbB-2蛋白表現陽性與 腸型或廣泛型胃癌預後無關。這些發現顯示,在胃癌病例中,c-erbB-2蛋白表現與腫 瘤組織型態的不同有關。而c-erbB-2蛋白的存在與否,似手僅與轉移潛力有關。 Expression of the c-erbB-2 oncogene protein was investigated by immunohistochemistry in 149 cases of primary gastric cancer. Positive staining, regarded as on indication of gene amplification, was evident in 38(25.5%) of the tumors. Of various clinicopathological factors considered , a statistically significant difference in association with frequency of expression was noted only for the histological type(39.3% positive staining in intestinal type, 15.4% in diffuse type, P<0.01), but not for age, sex, tumor site, gross type, size, depth of invasion, node metastases , and TNM stage. However, in seven of ten metastatic lymph nodes, the cancer cells stained positive for c-erbB-2, even when the primary tumor stained negative. This finding suggests that c-erbB-2 overexpression may be associated with the metastatic potential of gastric cancer. The 2-year survival rates of patients with positive and negative c-erbB-2 protein were 44% and 56%, respectively, which is without statistical difference. In subgroup analysis, erbB-2 expression was not associated with poorer prognosis in the intestinal type or the diffuse type gastric cancer. These findings indicate that, in the case of human gastric cancer, expression of c-erbB-2 protein is correlated with tumor histological type, and that the presence or absence of c-erbB-2 protein may serve as on indicator for potential of distant metastases.
