Abstract
The health care of chronic disease need combine with patient self-management for improving patient''s lifestyle and health behavior and finally improving patient''s disease and health status. In traditional self-management, patients record their health behavior, vital signs, medication and healthcare encounters on paper or by word processing software. These recording manners may result in difficult to maintain or manage data. This study was to build a personal health record application to help patients record their vital signs, health behavior and message or interact with case managers, and finally assist patients in actively involving their health care and improving their health status.
