Abstract
本研究採調查法從(一)居家照護需求的推估，(二)居家照護醫療照護費用之分析兩部 分，探討居家照護納入全民健康保險之可行性。結果發現：(一)目前需要居家照護之 人口，從醫院病人推估，需求率為5. 0%~8.9%；從社區民眾推估，60歲以上人口之需 求率為0.79%。(二)就實驗組(正接受居家照護者)與對照組(合乎居家照護條件的住院 病人)之實際費用比較，對照組為實驗組的9.1倍多。為減少長期住院所造成的資源浪 費，未來應建立慢性病人後續服務體系，而居家照護是重要方法之一，若能將之納入 全民健康保險體系，政策商也應有配合的措施。 This study is an investigation of projected needs of home care and a comparative analysis of the costs in home care and in medical care in order to examine the feasibility of home care in a national health insurance system. The results show 1) that the demand rate for the people who needed home care projected from patients in the hospital was between 5.0% and 8.9%; the demand rate projected from community population over 60 years of age was 0.79%; 2) The cost for the standard group staying in the hospital who met the criteria for home care was 9.1 times higher than that of the experimental group receiving home care . It is recommended that home care be inaugurated as an important way of a future follow-up services system for chronic patients in order to minimize waste in medical costs for those matching home care criteria. It is further recommended that there should be corresponding measures in health policy.
