Abstract
Chilaiditi sign, also called hepatodiaphragmatic interposition of the bowel, is an unusual incidental finding on the routine chest radiograph and manifests as subdiaphragmatic air. This finding is usually benign but is often mistaken as pneumoperitoneum if clinicians are unaware of this possible etiology. On the contrary, radionuclide images of such incidents usually present as positive findings with increased tracer uptake in the bowel. However, we recently encountered an atypical FDG PET finding of a Chilaiditi sign revealing right subdiaphragmatic crescent photopenia only. Although unusual, this case may prove to be a theoretically possible finding. It also demonstrates that such findings should prompt the consideration of including the Chilaiditi sign in the differential diagnosis, and not be routinely regarded as an artifact of FDG PET image acquisition or reconstruction.
