Abstract
膀胱炎症性偽腫瘤是一種相當罕見的疾病，好發於小於四十歲以下的病人及女性。患 者多因血尿接受檢查而被發現。雖然它是一種良性的疾病，且在接受經尿道膀胱腫瘤 切除後多不再復發，但仍有可能病人會因為被診斷成惡性腫瘤而接受了傷害性較大且 不必要的膀胱部份切除或全切除。泌尿科和病理科醫師必須知道這一種疾病的存在以 避免誤診。我們報告一位三十六歲的男性因為一個月內陸續發生了幾次的血尿和排尿 疼痛而就診。膀胱鏡檢查發現一個位於膀胱後壁的腫瘤。切片結果為慢性膀胱炎。經 內視鏡手術切除該腫瘤後，病人症狀消除。經過十五個月的追蹤及接受膀胱鏡和電腦 斷層檢查，發現腫瘤並無復發現象。最後病理診斷為膀胱的炎症性偽腫瘤。 An inflammatory pseudotumor of the urinary bladder is a rare disease. Although it is a benign condition, it may be misdiagnosed as a malignant neoplasm which might be treated too extensively. We report a case of an inflammatory pseudotumor of the urinary bladder in a 36-year-old man who presented with a 1-month history of painful urination and intermittent gross hematuria. Cystoscopy found a 2.5×1.5×1.5-cm polypoid tumor in the posterior wall of the urinary bladder. Biopsies yielded chronic cystitis. Transurethral resection of the tumor was performed. The pathological results showed a submucosal tumor composed of spindle cells proliferating in a vascular myxoid matrix infiltrated by lymphoplasma cells and eosinophils, which was suggestive of inflammatory pseudotumor. The patient 's symptoms significantly improved after surgery, and he has been followed up for 15 months. Cystoscopy and pelvic CT have demonstrated no evidence of tumor recurrence.
