關鍵字: | 腹膜透析;生命態度;peritoneal dialysis;life attitudes | 公開日期: | 2009 | 期: | n.2 | 起(迄)頁: | 191-209 | 來源出版物: | 長期照護雜誌,v.13 | 摘要: | 研究目的 本研究旨在描述並分析長期腹膜透析病人的生命態度。研究方法 採立意 取樣,選取中部某醫學中心16位接受腹膜透析病人為對象,進行兩次半結構性焦點團 體訪談,經參與者同意全程錄音、錄影,並將言語表達訊息轉錄為逐字稿,採用內容 分析法進行資料分析。研究結果 歸納出病人的生命態度共四個主題與十個次主題 (1)維持生理功能-「順應生理退化的事實,尋找治療方法」、「注重身體的安適, 調整生活的步調」、「體驗醫療的益處,期待有更進步的生技」 (2)調適心理壓力 -「緩解衝擊,尋獲存在的定位」、「失落健康,思辨親友的接納」 (3)考量經濟 能力-「衡量能力的消長,續承擔生產者角色」、「權衡物質與生命的輕重」 (4) 反思人生經驗-「保握現在,活出美好」、「對內思考生命尊嚴與品質」、「對外以 他人經歷相互作借鏡」。研究結論 腹膜透析造成病人生活改變,可由轉換看待生命 的態度,重新賦予疾病衍生的積極意義,進而建立有尊嚴的人生 唯部分個案確實尚 需要健康照顧提供者引導,以建立正面生命態度。期盼本研究結果可作為推廣腹膜透 析時的參考資料,藉由周全的介入措施與整合照護模式,協助腹膜透析病人建立正向 的生命態度。 The purpose of this pilot study was to describe life attitudes of peritoneal dialysis patients in a medical center of central Taiwan. Sixteen patients were recruited in two focus groups and received semi-structured interviews, which were taped and video-recorded. Based on the method of content analysis, four themes and ten related sub-themes emerged from interview data. (1) Maintaining physical function: Seeking for a comprehensive treatment in the progression of disease, focusing on physical comfort and coping lifestyle, and experiencing medical benefit in eagerness to a more advanced biochemical technique. (2) Adaptation of psychological disturbance: Identifying the acceptance of relatives while losing health, finding out existence when facing the impact. (3) Measurement of finical ability: Examining the value between material and life, and playing a producer role after evaluating one's ability. (4) Exploration of life experience: Cherishing the present and living well, having insights about the dignity and quality of life, learning from others. The modality of peritoneal dialysis makes patients change life pattern. However, they could stand upon their dignity while exchanging attitude of life and transforming illness. In addition, some of participants need healthcare providers guide in establishing the positive life attitude. By the comprehensive approaches and the integrated caring mode, peritoneal dialysis patients can establish positive life attitudes and receive an appropriate medical service. The results provide references for healthcare providers to promote peritoneal dialysis. |
