標題: | 行政院國家科學委員會專題研究計畫成果報告:醫學生的壓力、因應與身心健康之探討-醫預課程與基礎醫學課程階段之貫時性比較分析 | 作者: | 呂碧鴻 | 關鍵字: | 醫學生知覺壓力;大學生身心適應困擾;心理健康;社會網絡;perceived medical school stress;college student’s adjustment problem;mental health;social network | 公開日期: | 2005 | 出版社: | 臺北市:國立臺灣大學醫學院社會醫學科 | 摘要: | 醫學教育內容豐富,醫學生學習過程相當艱辛。許多研究顯示醫學生 知覺到的壓力是影響心理健康,甚至未來罹患精神疾病的重要因素。本研究 目的是:1.瞭解醫學生知覺的壓力內容與程度及其影響因素。2.分析課程性 質與內容,教學方式在醫學生的壓力知覺上佔有之角色。3.探討因應策略與 社會支持中介變項之影響效果。4.探討壓力導致之結果及釐清壓力與心理不 健康之預測因素,以便及早輔導與介入。 以7所醫學院之二年級醫學生為對象,進行兩年之追蹤研究。以問卷測 試進行資料蒐集,包括人格特質、生活事件、壓力測量、因應策略、社會網 絡資料、學業資料、健康狀態、學習態度等。壓力測量採用醫學生壓力知覺 量表及大學生身心適應調查表進行。 第一年研究共有680位醫學生,壓力測量結果顯示93.8%學生知覺醫學 院壓力,其中以學問需求之壓力為主;一般身心適應壓力,尤其時間管理困 擾,亦是重要壓力源。社會網絡及支持良好。影響醫學生壓力知覺的因素有 自尊評價、生活事件及同儕關係,可解釋18.7%之變異量;影響身心適應壓 力因素有社會支持、心理健康及自尊評價,可解釋30.9%之變異量。心理健 康評估中,自覺不快樂者130人(19.2%),有憂鬱情緒者佔374人(55%), 焦慮者356人(52.4%),失眠者222人(32.6%)。心理健康之預測變項 以身心適應困擾為主(解釋變異量21.6%),其他為自覺身體健康狀態、快 樂狀態、生活事件與自尊評價(共可解釋32.5%之變異量)。而醫學生壓力 知覺對心理健康並不具顯著預測效力。所以第一年之研究結果顯示醫學生心 理健康議題須重視,一般身心適應之困擾,尤其時間管理困擾須及早介入輔 導,以便因應未來艱辛的醫學專業學習及確保心理健康。 A considerable number of researches have shown that perceived medical school stress(PMSS) is hazard to health and predictive to psychiatric morbidity. As more courses of medical humanities and problem-based learning strategy are two main innovations in the medical education reform, their impact on students is yet and worthy to explore. The purpose of this study was : 1. to understand the perceived medical school stress and the relating factors ; 2. to explore the role of different courses and different teaching strategies playing in PMSS ; 3. to analysis the interaction among stress ,coping ,social support and outcome; 4. to find out the predictors of PMSS and psychological morbidity for early intervention. We conducted a longitudinal study using questionnaire survey to the second year medical students in seven medical schools. The questionnaire included demography, life events, social network and measurements of self-esteem, health status and stress. Stress was assessed by PMSS scale and the College students’ Adjustment Check List (CSACL). Total 680 medical students participated in the first year study. 93.8% of the students perceived the medical school stress and academic demands were the most serious concern. Time management problem in the CSACL was also a significant stressor. Most students had good social relationship and support. There were 130(19.2%) students felt unhappy, 374(55%) students had depressive mood, 356(52.4%) students had anxious mood and 222(32.6%) students had insomnia. Multiple regression analysis showed that self-esteem, life events and peer relationship accounted for 18.7% of variance in PMSS; but social support, mental health and self-esteem accounted for 30.9% of variance in CSACL. The predictors for mental health were adjustment problems in CSACL (explained 21.6% of variance) and perceived health status, emotional status, life events and self-esteem. Overall those variables accounted for 32.5% of variance. However PMSS didn’t show predictive power to mental health. Conclusion: This first year study indicated that student’s mental health is an important issue need to be addressed. Adjustment problems, especially time management, should be early identified and intervened in order for them to cope with the future painstaking medical profession so as to keep their personal well-being. |
