Acute suppurative thyroiditis is a rare diseasse, caused by pyogenic organisms or rarely by nonmicrobial infections. Because of its rarity and sometimes it can be mimicked by other disorders, definite diagnosis is very important to prevent the exacerbation or prolongation of the course. We reported a case diagnosed by fine needle aspiration cytology and treated with antibiotics with good response. The cytology pictures were the presence of polymorphonucleated leukocytes, histiocytes and degenerated follicular cells in the acute phase; mature lymphocytes, plasma cells, fibrocytes and regenerating follicular cells in the convalescent phase. We think the best way to establish the diagnosis of acute suppurative thyroiditis is fine needle aspiration to get material for cytology and culture, accompanied with clinical picture or scintiscan if there is no evidence of follicular cells or collid in the smears.
