METHODS Our study followed up 142 children with EV71 CNS infections--61 with viral meningitis, 53 with severe CNS involvement including encephalitis, polio-like syndrome and encephalomyelitis, and 28 with cardiopulmonary failure after CNS involvement. Around 2.85 years ( range 1.0-7.39) after onset of disease, the children received physical and neurological examinations . Additionally, children below six years old were given the Denver developmental screening test and those above four years old an intelligence test. RESULTS Nine (56%) of the 16 polio-like cases and 1 (20%) of the 5 encephalomyelitis cases had sequelae involving unilateral limb weakness/ atrophy. Eighteen (64%) of the 28 cases with cardiopulmonary failure after CNS involvement later had limb weakness/ atrophy, 17 (61%) required tube feeding, 16 (57%) required ventilator support and 5 (18%) had hypoxia encephalopathy. Delayed neurodevelopment was found in 1 (2%) case with severe EV71 CNS involvement and in 21 (75%) cases with cardiopulmonary failure after CNS involvement (p<0.001). Their intelligence quotient was affected by cardiopulmonary failure after CNS involvement (=-15.5, p=0.003); the age of onset <2 years significantly affected the verbal comprehension in cases of CNS involvement alone (=- 11.8, p=0.009). CONCLUSIONS EV71 CNS involvement with cardiopulmonary failure may result in neurological sequelae, delayed neurodevelopment and reduced cognitive functioning. Those with CNS involvement alone did well in neurodevelopment but had a reduced verbal comprehension if the age of onset was <2 years. ( ClinicalTrials.gov number, NCT00172393) |
