Abstract
Recanalization of chronic total occluded coronary vessels has prolonged benefit. Percutaneous transluminal coronary angioplasty (PTCA) has been reported to achieve a relatively lower primary success, and higher restenosis, rate. Other techniques such as excimer laser coronary angioplasty (ELCA) may be helpful. Herein is reported a successful ELCA treatment for a chronic total occlusion following PTCA. The patient, a 75-year-old female with a 95% discrete eccentric stenosis in the middle portion of the right coronary artery, was treated by PTCA. An acute occlusion which developed after PTCA was unable to be reopened despite repeated attempts to cross the balloon catheter over the occluded segment. Six months later, the patient was admitted again because of persistent chest discomfort. After failure to advance the balloon catheter across the occluded segment, excimer laser was tried and resulted in a well-dilated right coronary artery. Six months after the excimer laser angioplasty, coronary angiogram did not reveal restenosis. Thus, excimer laser should be tried in symptomatic chronic total occlusion which can be crossed with a guidewire but not with a balloon catheter. The role of this procedure in acute occlusion following PTCA deserves further evaluation, if there are no contraindications.
