OUTPATIENT PHARMACY SERVICES
Outpatient pharmacy dispenses about 20,000 prescriptions per day. The primary responsibilities of pharmacists include filling and verifying the prescriptions, and ensure the accuracy of medications dispensed. Patient medication counseling is provided to about 5% of outpatients Our future plan includes remodeling of the outpatient pharmacy, reform of dispensing process, expanding the use direct to consumer packages, and application of computer and bar code in verifying prescriptions and medications, as well as patient counseling services.


INPATIENT PHARMACY SERVICES
Inpatient pharmacy provides services to all the inpatients. In addition to traditional pharmacy services, such as supplying medications to the wards, verifying and filling prescriptions and ensure the accuracy of medications dispensed, all the pharmacists also provide clinical pharmacy services routinely. These include drug therapy monitoring, patient-specific therapeutic consultations to health care practitioners, adverse drug reaction monitoring and reporting, and patient medication education. Specialized clinical services are integrated into care units as designated pharmacists round with the medical team every day. Through direct intervention, pharmacists may assist in a great deal of therapeutic decision making and therapeutic drug monitoring activities. Our future plan include implementing point-of-care system, application of computer and bar code in verifying computerized prescriptions and medications, as well as expanding clinical pharmacy services.


CLINICAL PHARMACY SERVICES
Currently, clinical pharmacy services provide pharmaceutical care to all intensive care units, nephrology and hematology-oncology ward. Clinical pharmacists attend rounds with medical team on daily basis and give recommendations to ensure appropriateness and safety of drug therapy. They are preceptors of Pharm.D. or M.S. of Clinical Pharmacy students in advance pharmacy practice experiences, and are responsible for training new pharmacists. In addition, clinical pharmacists join ICU committee and infection control committee to improve medication use in these two areas.


EMERGENCY ROOM PHARMACY SERVICES
Emergency Room Pharmacy runs 24 hrs all year round to serve emergency patients and outpatients from the Jing-Fu VIP and dialysis clinics. We also fill inpatient medication orders after office hours, and are responsible for the floor stock medications of the emergency department and several examination departments. In addition to essential pharmacy services (such as verifying, filling prescriptions, and ensure the medication accuracy), we document and report adverse drug reactions (ADR). We are responsible for evaluating use of all antidotes, reviewing potential drug related problems and managing special antidotes offered by the government. We serve medication educations for all patients discharged from emergency department, as well as medication counseling for patients and all medical staff, including medication reconciliation and identifying. We optimize pharmacotherapy regimens utilizing current evidence-based recommendations and serve therapeutic drug monitoring for medications with narrow therapeutic range. Our future plan are expanding and providing comprehensive pharmaceutical care to the emergency department, cultivating pharmacists with emergency department specialist, and providing educational programs for patients, College of Pharmacy students and hospital staff.


ONCOLOGY PHARMACY SERVICES
Oncology pharmacy compounds chemotherapeutic agents for outpatients and inpatients. 14 pharmacists and 1 technician are responsible for 300 chemotherapy orders each day. Our service is year round, with half a day service on holidays. We’ve also adopted an automated robotic system named CytoCare® since 2010 to take up around 20% of our current workload. We have two clinical pharmacists who do patient monitoring on the floors as well as adverse drug reactions follow-ups. We are currently developing a computerized chemotherapy protocol-based prescription system, in the hope that more accurate prescription information can be passed on to the pharmacists and nursing staffs, so as to curtail medication errors.


PARENETERAL NUTRITION SERVICES
Our Parenteral Nutrition Section serves the inpatients and home PN patients in the hospital. Routine quality assurance procedures to improve patient safety include patient monitoring and verifying order before compounding, daily compounding accuracy test, and environmental controls, monthly anemometric test, as well as monthly sterility test and particle counting. Our future plan includes expanding the commercial RTUs (ready-to-use) PN formulae and upgrading the current CPOE system to further improve physician’s prescribing and patient care quality.


SECTIONS OF INVENTORY CONTROL
Inventory Control Section handles drug purchasing and distribution to the dispensing units, maintains drug costs, notify drug related issues, and manage controlled drugs and investigational drugs. In order to improve our inventory system, we haveplan to implemented the WMS (Warehouse Management System, which utilizes Auto ID Data Capture technology, such as barcode scanners, mobile computers etc, to efficiently monitor the flow of products) to upgrade the inventory control system from currently system to the electronic inventory control system in the following year 2009.


DRUG INFORMATION SERVICES
Our Drug Information Center provides drug information service for health professionals and the public. We also maintain NTU Hospital Formulary, play an important role in the Pharmacy and Therapeutics Committee (P&T committee), arrange in-service training program, provide externship for pharmacy students, arrange on-the-job training for pharmacists from other hospitals, report adverse drug reactions to Department of Health, and evaluate controversial prescriptions. Our future plan includes expanding E-patient information and restructuring NTUH pharmacy website.


NTU CHILDREN’S HOSPITAL PHARMACY SERVICES
The children’s hospital pharmacy provides services to patients under 18 years of age. Our mission is to improve patient’s quality of life and to provide good patient education to the children and the parents. The primary responsibilities of our pharmacists include verifying and filling prescriptions, and ensure the accuracy of medications dispensed. Patient medication counseling is provided in both inpatients and outpatients. Our inpatient service include verifying the physician’s orders, drug therapy monitoring, patient-specific therapeutic consultations to health care practitioners, adverse drug reaction monitoring and reporting, and patient medication education. In addition to traditional pharmacy services, the pharmacy division also provides a variety of clinical pharmacy services to our patients. Specialized clinical services are integrated into care units as designated pharmacists round with the medical team every day. Through direct intervention, pharmacists may assist in a great deal of therapeutic decision making and therapeutic drug monitoring activities.


PROMOTION OF PHARMACY EDUCATION AND CONTINUING EDUCATION
As an affiliated hospital of the National Taiwan University, department of pharmacy, in cooperate with school of pharmacy and graduate institute of clinical pharmacy, provides pharmacy practice training program for students from school of pharmacy or graduate institution of clinical pharmacy, as well as pharmacists form other hospitals or community pharmacies.
