Description
本計畫結合胸部電阻斷層成像(EIT)與胸腔超音波,提供即時、非侵入性的呼吸力學、區域肺容積、呼氣末肺容積(EELV)及橫膈膜功能監測。在第1與第2年,針對中重度急性呼吸窘迫症候群(ARDS)患者,透過EIT輔助的吐氣末正壓(PEEP)調整,探討肺部超音波在評估肺部塌陷與過度擴張中的應用價值,並結合橫膈膜超音波、pendelluft效應、氧氣消耗(V̇O2)及炎症生物標誌(如IL-6、IL-8)分析自我誘發性肺損傷(P-SILI)的預測能力。第2與第3年將聚焦於自發性呼吸試驗(SBT),透過EIT的呼氣末肺阻抗變化(EELI)、胸腔超音波及氧氣消耗數據,建立拔管成功的預測模型。預期成果包括發表研究數據及臨床操作指引,提升ARDS患者的治療精準度與拔管成功率,為重症醫療帶來貢獻。
