2015-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/647230摘要:本研究乃透過運用基隆市社區整合式篩檢世代(Keelung Community-based Integrated Screening Cohort, (以下簡稱KCIS)」進行之創新擴散研究。本創新計畫自2000年起至2009年底結束,統合五項癌症與三項慢性病與成為創新的社區整合式篩檢。此創新整合式篩檢服務強調以家庭為基礎。內容包括:疾病篩檢、異常個案轉介與問卷調查,並針對篩檢異常與高危險族群個案進行轉介及追蹤建檔管理、面對面衛生教育與健康諮詢服務。此研究之目的為探討KCIS在十年間如何被民眾接受與擴散,並理解此模式對於民眾健康貢獻。並以糖尿病為例,探索慢性病家族聚集及跨世代之擴散經驗,歸納分析個人因素、環境因素與家庭因素等,來探索相關因素間對於糖尿病家族聚集與跨世代發展之關聯性與危險性。本研究預期能同時能達到探索社區整合式篩檢、轉介、管理與分析的目的,呈現此創新模式十年間在社區的擴散真實狀況,以及相關變數如何影響此創新被擴散;最後能理解糖尿病家族聚集跨世代現象,可作為未來國家社區篩檢推動之重要依據。<br> Abstract: This study was based on diffusion of innovation theory (DOI) to analyze the secondary database of Keelung Community-based Integrated Screening Cohort (KCIS). The current study aims to realize and retrospect the adoptive pattern of KCIS cohort from year 2000 to 2009. KCIS program screened for 3 chronic diseases and 5 cancers as the innovative services, inviting all family members in communities. KCIS datasets collected the anthropometric measures, biochemical assays and lifestyle questionnaires during screening process under the approval of the city government.This study will use diabetes mellitus as a DOI model to explore the association and related factors for familial aggregation and trans-generative diffusion experience. The integrated screening, referral, management and analysis will be studied to reveal the condition of “real world” in the 10 year period. The impact of significant factors for diffusion will be found. The familial aggregation and trans-generation will be explained for diabetes mellitus.
