2018-08-012024-05-14https://scholars.lib.ntu.edu.tw/handle/123456789/655811背景:末期腎臟病是近年來全世界共同面臨的公共衛生重要議題,透析治療造成龐大的經濟負擔。根據2015年美國腎臟登錄系統報告顯示,台灣末期腎臟病之發生率為每百萬人口458人,盛行率為每百萬人口3,138人,居全球之冠。2015年台灣地區十大死因中,腎炎、腎病症候群及腎病變排名第九。在慢性腎臟病時期,若能及早控制,可延緩進入末期腎臟病的時間。慢性腎臟病依腎絲球過濾率可分為五期,除了飲食、運動及良好的服藥習慣外,若病人可即時掌控自己的健康狀況,不僅對病況控制有幫助,也可減少因病況改變所需的額外費用。目的:透過引進新興科技- 健康管理平台結合穿戴式裝置,導入病人的自我管理,並作成本分析。研究方法:以慢性腎臟病病人為受試者,預計實驗組30人,控制組30人。針對實驗組以健康管理平台結合穿戴式裝置作為介入90天,透過手機行動載具記錄飲食及運動數據,並成立通訊軟體群組鼓勵受試者。收集自主管理量表分數及各項生理、檢驗指標,進行前後測驗的比較。並分析180天內兩組之健康照護成本。預期效益:本計畫若獲得補助,將幫助受試者建立自我管理的健康管理模式,使慢性腎臟病病人減緩惡化至末期腎臟病的速度。Background: End stage renal disease (ESRD) is a global public health issue. The dialysis therapy brings huge economic burden to many countries. According to the report of 2015 United States Renal Data System (USRDS), the incidence rate of ESRD is 458 people per million populations. And the prevalence rate is 3,138 people per million populations, which is the highest one in the world. Chronic kidney disease (CKD) can be classified into five stages according to glomerular filtration rate. Whatever the stage it is, patients should keep healthy habits about diet and exercise and good adherence to medication. If patients are able to monitor their lifestyle, it not only promotes health, but also reduces the cost of health care. Aim: By application of wearable devices and health management platform to build self-management model in CKD patients and analyze the cost of healthcare. Material and Methods: In our study, we plan to recruit 60 CKD patients as trial subjects. The experimental group is composed of 30 participants, and the control group 30 participants. The experimental group received intervention with wearable devices and health management platform for 90 days. Patients record their own diet diary by a smart phone application, and their exercise-related data are collected by wearable devices. Then, all the collected information will be upload to health management platform. We also create a LINE group to encourage exercise in the experimental group. We compare the scores of self-management sheet and physical and laboratory examinations before and after the intervention. We also analyze the cost of healthcare within 180 days.Anticipated Results: If the grant will be approval by reviewers, we will help CKD patients to keep in healthy habits about diet and exercise and good adherence to medication.
