2014-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/646728摘要:慢性肌腱病變在臨床上,尤其是復健醫學上,是一種常見的疾病及疼痛的來源,目前臨床上雖然有諸多治療方法,效果卻常常不一致或不理想。本研究計畫希望發展出一種創新的治療方式 – 基因療法,來根本解決這種惱人的疾病,我們要發展的治療方式是基於目前常用的血小板濃厚液加以改造,包含以下重要步驟:(1)由周邊血液抽取定量的靜脈血,(2)離心取得血小板濃厚液層(PRP),(3)以超音波 PRP層細胞導入TGF‐beta plasmid DNA,(4)將改造後的 PRP層細胞注入肌腱與肌肉交界處組織,希望這些被轉殖的細胞直接分泌肌腱修護需要的 TGF‐beta蛋白質因子,達到修復的目的。本計畫將以三年時間,發展這些技術所需的基礎技術,為臨床應用準備,並探討治療所引起的可能細胞內分子機轉。<br> Abstract: Chronic tendinopathy is a commonly seen disorder, especially in rehabilitation medicine, and is responsible for substantial morbidity and pain of victims. Although various treatments existed, their effects are sometimes inconsistent and frustrating. The main purpose of this proposed study is to develop a novel treatment method, i.e., gene therapy, aiming at curing this annoying disorder. Our basic approach was modified from the new treatment modality, the platelet‐rich plasma injection for tendinopathy. The main procedure includes the following steps, (1) get adequate venous blood from peripheral vessels,; (2) extract platelet rich plasma layer by spinning; (3) transfect therapeutic DNA, in the current proposal, TGF‐beta plasmid; (4) ultrasound gene transfection of therapeutic plasmids to the soft tissue between tendon and muscle, and hopefully, the transfected plasmids will exert their therapeutic effect on target tendons. We plan to use three years to develop the basic techniques necessary for future human trials, and probe the possible intracellular signal pathways of the proposed method.
