2020-06-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/653300摘要:普遍認為,人們在健康風險上的制度化互助,也就是各種類型的公共資助健康體系,是由社會連帶所支持的照顧責任集體分配。在台灣,儘管全民健保制度看來已相當成熟,甚而成為台灣形象之一部,但現有對健康連帶感作為一種社會倫理精神之實證研究仍相當有限。面對人口老化、經濟停滯和新藥新科技,各國健康體系的財務負擔沉重,也對支持健康體系的社會連帶構成巨大挑戰。若社會連帶並不存在,被迫改革的健康體系可能扭曲原初在健康風險上互助的目的,甚而出現對特定族群之汙名或排外情緒;若社會連帶果真存在,則會是維繫健康體系永續經營的關鍵因素。本研究旨在探知台灣社會健康連帶感的內涵與形式,以及人們理解中照顧責任應該如何分配,以補足健康連帶感的知識缺口,並作為未來健康體系改革之參考依據。本研究以台籍移工與台籍留學生為研究對象,採用質性訪談研究法,透過比較的觀點,訪談其在外國與本國的健康體系使用者經驗、對健康風險互助、健康照顧責任分配之理解判斷。<br> Abstract: People`s institutionalized mutual assistance in sharing health risks, in a variety of forms of publicly funded health systems, is a collective (re-)allocation of responsibility for care which is presumed to be sustained by social solidarity. Despite the fact that the National Health Insurance in Taiwan has become a stable system and enjoys an overall high satisfaction rate among the people – even becoming an element of Taiwanese identity – health sector solidarity as a socio-ethical sentiment has received limited empirical research attention. This knowledge gap warrants further investigation because health systems are facing challenges such as population aging, economic stagnation, and advancing medical technologies and new drugs, along with increased public expectations. If health sector solidarity does not exist, unjustified health system reforms might distort the original goals of sharing health risks, and even encourage xenophobic sentiment and stigmatization against certain populations. On the other hand, if health sector solidarity does exist, health systems and reforms could be better sustained. This project, therefore, aims to empirically investigate the forms and contents of health sector solidarity in Taiwan. It contributes intellectually to the solidarity studies literature as well as practically to future health reform.
