2014-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/643595摘要：家庭責任醫師制度是實施二代健保法的一個主要政策，目的在以有效的醫療模式，提升醫療品質。家庭責任醫師制度採論人計酬制，主要是由大型醫療體系所主導。制度的成功，在於參與家庭責任醫師制度的每一位醫師，能夠具有具體認知與確實執行的能力，家庭醫學專科醫師將是發展家庭責任醫師制度的主力。本計畫擬以兩年時間進行研究，探討當前的家庭醫學專科醫師養成計畫，能否提供家庭責任醫師制度的醫師人力，以及有效的家庭責任醫師執業模式。研究對象為台大家醫部之所有住院醫師，約 50 餘人。 第一年的重點在蒐集相關計畫之研究成果，由 6 位相關領域的專家，以修訂式大慧法訂定家庭責任醫師之能力基準，據以檢核國內家庭醫學醫師的訓練情形，以瞭解在培育家庭責任醫師的能力方面，所需要加強之處，再參酌類似以病患為中心之醫療屋的能力要求，訂定家庭責任醫師的能力項目與訓練計畫。 第二年的重點在以行動研究法，利用導生制度，探討家庭責任醫師能力之訓練與實作模式，並檢定其訓練成效。本計畫預期要訂定具體之家庭責任醫師的能力項目、訓練模式與評量基準，建立家庭責任醫師符合國情與社會習慣之各項能力的運作模式，以落實二代健保在醫療照護方面的主張。<br> Abstract: Family physician system is one of the major components of the second edition of the National Health Insurance in Taiwan. The purpose is to establish an effective primary care system to improve health care quality. The system uses capitalization for reimbursement and only major health institutes are motivated to participate. The success of the system depends on the awareness and competence of participating physicians. Family medicine specialists are the major force for the system, this research is to be completed in two years period of time. The study objectives are the family medicine residents at National Taiwan University Hospital. The first year is to invite 6 experts to use a modified Delphi method to establish the criteria for the related competence of the current family medicine trainees and to propose a modification plan for the current training program. The second year is to use the action research method to evaluate the effectiveness of the newly developed special training program from the previous year. Appropriate revisions will be made and the implementation model for each proposed skill will be investigated.
