2011-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/655136摘要:奈米粒子輔助之唾液腺組織特異幹細胞治療唾液腺功能缺損在臨床上是一個重大的問題,尤其是在台灣這個頭頸部癌症盛行的地區。唾液腺功能的缺損至今在臨床上並無有效的治療方式,患者因唾液腺疾病或是接受頭頸部腫瘤的治療所造成的唾液腺功能缺損會導致相當多健康的問題和生活的不便。現今創新發展的幹細胞治療可提供恢復唾液腺功能的可行治療。在發育完全的成熟體細胞,現今的研究証實仍有組織特異幹細胞存在於體內各組織器官。而這些幹細胞仍有其分化和再生的能力,可於體內修補和再生功能性組織。唾液腺也証實有類似體幹細胞的存在。在實驗中証實,以唾液腺微粒球的方式培養,可保有幹細胞的特性,並可分化成不同的成熟唾液腺細胞。然而,唾液腺微粒球複雜的細胞組成使得細胞的純化不易實行,而其相關的細胞治療條件也尚未完全釐清和確立。因此,本計劃將利用新發展的奈米粒子技術來輔助唾液腺微粒球的培養和純化。藉由篩選和培育唾液腺微粒球的技術,來純化並增殖具有分化能力的細胞,以做為細胞治療的來源。其次,在確定這些唾液腺微粒球的特性後,有關細胞治療的臨床條件將進一步探討。詳細的臨床細胞治療條件,將藉由動物實驗去模擬未來臨床運用的情形。而可能在細胞移植所造成的副作用,也將在條件中去嘗試和修正。此外,對於接受治療的唾液腺功能也將一併評估,以期了解唾液腺功能恢復之情形。而移植後的細胞和組織,也將進一步去探討其和原生組織之關係,希望藉由這個研究,使得唾液腺功能的細胞治療能再向前邁進一步,使其得以成為如骨髓移植般成熟的臨床細胞治療技術,以嘉惠廣大的病患。<br> Abstract: Nanoparticle-assisted tissue-specific stem cell therapy fordysfunctional salivary glandSalivary gland dysfunction is an important clinical problem, especially in the patientswith irreversible salivary gland damage caused by diseases or therapeutic managements.In Taiwan, the higher incidence of head and neck cancer results in a larger population withdysfunctional salivary gland. Nonetheless, until now, no effective management has beendeveloped yet. To restore physiological function of organs by providing cells withregenerative capacity might be a promising way. The current advance in cell therapybased on tissue specific stem cell render this strategy feasible. Tissue specific stem cellshave been well documented to exist in many different organs. These cells are proved tobe capable of differentiation and regeneration, which could be used in the repair andrecover of tissue function. In salivary gland, tissue specific stem cell has beendocumented to be isolated from mature salivary tissue. The spheroid formation of salivarycells, namely salisphere, is beneficial to stem cell isolation and maintenance. Nonetheless,the heterogeneous population of salisphere renders it difficult to be purified and regulatedfor desired differentiated status. Moreover, the condition of stem cell therapy towardsdysfunctional salivary gland has not been well defined yet. In this proposal, the aim is todevelop a novel approach to cultivate salivary progenitor cells. Assisted bynewly-developed techniques of nanoparticle, salispheres abundant in multipotent cellscould be enriched without sacrifice of viability and interaction of cells. In addition, theconditions of stem-cell based cell therapy for salivary dysfunction will be further defined.The procedure and the possible complications of salisphere transplantation will beexamined.
