2012-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/643040摘要:泌尿道感染是常見感染,並為常見敗血症原因,以及腎臟功能受損原因之一,近年因不當使用抗生素更增加許多抗藥性病例。若使用溫和方法治療感染,保護腎臟免於纖維化風險,改善不適症狀,對增進患者生活福祉很重要。中藥有千年歷史和溫和調理效用,近年來受到國際重視。八正散用於治療泌尿道感染出自「太平惠民和劑局方」,內有「車前子、瞿麥、萹蓄、滑石、山槴子、甘草、木通、大黃」。其中成分,有抑菌、免疫調節、抗氧化、抗發炎、抑制腫瘤和血管舒張作用。但關於泌尿系統保護作用研究較少。本研究有四部分:1八正散濃縮製劑和傳統水煎劑是否有同樣的療效,目前已有初步結果發現二者對於發炎時細胞激素的產生和脂質過氧化壓力都有相當抑制作用;未來將進行2了解各單方萃取物功效;3以HPLC的方法找出可能指標活性成分;4利用指標活性成分探討對於感染嚴重度改善,全身和泌尿系統免疫反應和過氧化壓力,預防腎臟結痂,改善膀胱感染不適症狀等方面之相關機轉。本計畫完成時,對於八正散改善泌尿道感染時不適症狀,改善腎盂腎炎發炎反應,防止腎臟結痂效用和機轉一定有所了解,對此中藥效用推廣,定有助益。<br> Abstract: Urinary tract infections (UTI) represent a major clinical problem. Although mortality rates are low, morbidity and the decrease in the quality of life are huge. The long term complications occur such as chronic renal failure. These complications are mainly produced following the formation of renal scarring in patients with UTIs. Patients suffered from UTIs complain of frequency and urine retention. These symptoms impair their quality of life. Antibiotic treatment is the gold standard for the most UTI cases. However, with spreading antibiotic resistance alternative treatment options are needed. Ba Zheng Son(BZS), an important traditional Chinese medicine formula for the treatment of UTIs, contains Chinese plantain, large pink, talc, rhudarb, cape jasmine fruit, herba polygoni avicularis, caulis clematidis, slender licorice root, and common rush. The components of BZS are potent in anti-diabetes, anti-oxidants, anti-inflammation, antiviral agents, antibacterial agents and vasorelaxants. However, to date, there have been few studies on the mechanism of BZS on UTIs and associated complications.There are four parts in our project. First, we have investigated the biological effects of traditional herbal decoction and the commercial extract of BZS. There are two different forms of BZS in Taiwan. One is traditional herbal decoction, and the other is commercial extract. Our preliminary data found the biological effects of these different forms had the same anti-inflammatory effects in the mice UTI model. . Secondly, we will determine the individual effects of the extracts. Thirdly, we will search the contents of the bioactive constituents. The final step, we will investigate the mechanism of bioactive constituents separately.We will evaluate whether traditional herbal decoction, the commercial extract, the individual extracts, and search the contents of bioactive constituents. We will explore the compound of the components of BZS attenuate the inflammatory severity of acute pyelonephritis in rats, and explore the possible mechanism. To further evaluate their protective properties, we will determine the protective effects against bacterial induced renal fibrosis signals. The contractions of isolated rat detrusor strips evoked by field stimulations (EFS) or by exogenous agonist and immunoblotting for the detection of protein expression in the bladder and kidney will be observed in the short (1 h) and long (24 h) term. We will test the hypothesis of the possible improvement of the detrusor dysfunction by BZS and discuss the possible mechanism.Taken together, we hope these experiments will support a therapeutic effects and mechanism of BZS for the treatment of human urinary tract infection associated uncomfortable symptoms and severe complication such as acute pyelonephritis, renal fibrosis and sepsis, and further understanding the anti-inflammatory and anti-oxidant mechanism of these drugs.泌尿道感染腎盂腎炎腎臟纖維化八正散urinary tract infectionpyelonephritisrenal fibrosisBa Zheng SonThe Effects and Mechanism of Ba Zheng San Prescription on the Urinary Tract Infections (II)=八正散方劑治療泌尿道感染的效用及機轉之探討(II)
