2014-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/644634摘要:近年來,腦部腫瘤病患基於各種考量而選擇採取立體定位放射手術的比例,已出現大幅成長。當神經外科醫師評估腫瘤治療效果時,往往是依據病患腫瘤的體積變化作為主要的評估標準,而放射手術後腫瘤的體積變化,屬漸進式,未若開顱手術切除來得直接,因此需要長期追蹤。如何發展一套可更為精確地測量腦部腫瘤體積的方法,以作為評估病患放射手術成效的依循標準,藉以做出後續治療決策,實為一大要務。 腦部腫瘤體積的測量,往往採取人工測量方式,亦即藉由電腦斷層或核磁共振等電腦影像,估量腦部腫瘤的長度、寬度以及高度,以計算出腫瘤的體積。有時甚至只測量最大徑。然而,人工測量方式係屬單一向度,於是在對立體的腫瘤進行測量時,極易因為角度的些微變化,而出現不一致的測量結果。因此傳統人工測量方式的應用,受制於人為變數,致使醫師無法在特定期間內持續追蹤腫瘤體積變化,進而評估放射手術的具體成效。 有鑑於此,我們擬研發一套在影像上計算腦部腫瘤體積的演算法,以提升腫瘤體積量測的精準度及一致性。本計畫的主要目的,就是整合治療前和治療後追蹤的影像,以及我們過去發展影像自動評估系統的經驗,發展一套自動評估腦部腫瘤體積變化的演算法和應用程式,作為臨床醫師的決策輔助工具。 <br> Abstract: In recent years, the number of brain tumor patients undergoing radiosurgery is increasing. When neurosurgeons evaluate the post-radiosurgery effects for clinical decision-making, the tumor volume change is usually taken as the single most important factor. Contrary to microsurgical resection, tumor volume change after radiosurgery is a gradual process. Therefore, it’s essential to develop a more precise method for tumor volume estimation, which can be used as the supportive tool for clinical decision-making. Usually, manual measurement is used for brain tumor size estimation. Based on medical imaging, i.e., MRI and CT, the length, width and height of the tumor are used for tumor volume estimation. Very often, only the maximum dimension was measured and recorded. Nevertheless, the result of manual measurement is prone to errors, due to factors like gantry angle and arbitrary direction of measurement. We can say that the conventional manual measurement is very limited and thus makes it difficult to follow the volume change and to evaluate the treatment effects. In light of this, we are planning to develop an algorithm for automated volumetry to improve precision as well as consistency.
