2011-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/655138摘要:末期醫療照護之醫學教育強調學生應具備末期照顧於「知識、態度及臨床技能」三個面向的能力,本研究團隊過去幾年已針對此三面向的能力,著重在不同形式之教學教材內容的研發設計,發展出醫學院大學部學生生命末期醫療照護之教育目標、主題以及以病人為中心之多媒體互動教學法,也實際進行了教學與評值。同時更發現對於生命末期照護以病人及家屬為中心的醫護的臨床專業技能、溝通與團隊合作、關懷、終身學習、倫理以及批判性思考之核心能力,必需建立一套規劃完善且有系統之教學策略與實際操作之客觀評量方法,才能真正落實生命末期照護强調之臨床實務與技能之審核,在雙向互動的教學情境下,提供態度與技能更好的補救教學與評量。本子研究計畫據此並主要依據整合性研究計畫的目標,將以三年期間致力完成下列研究目的:1)以現有發展的本土化教材為基礎發展醫學院末期照護臨床核心能力的教學策略;2)以OSCE為主軸發展並建置適合國內末期醫療照護中「倫理決策」與「跨團隊溝通」兩大議題之有信效度之臨床核心能力評值工具與方法,藉此落實倫理決策能力與跨領域團隊的溝通合作知能;3)亦將檢視末期照護行為意向、相關影響因子,特別針對末期照護常見之「倫理」(ethical decision making)及「跨領域團隊溝通」(team communication)兩大議題,以修正更具適切性的多元化教學策略來增進末期照護能力;4)舉辦研習營與共識會議,進一步修正教學策略與評值工具,並推廣之。藉由以上研究目的的完成,建置醫護教育中末期照護教育完善的教材、教學策略與評值工具,並持續推廣以全面提昇國內末期照護的教學品質。<br> Abstract: End-of-life care in medical education is emphasizing the patient-centered and the learning in knowledge, attitudes and skills. After the endeavors in our previous studies, the improvement in the curriculum of end-of-life care in knowledge, attitudes and skills has been noted.However, to establish better educational system, it is necessary to develop comprehensive teaching strategies and evaluational tools toward the clinical competency communication, team work, compassionate and ethical decision making.The three-year period study will be aimed to: 1) design teaching strategies of clinical core competency based on the teaching materials previously established, 2) develop evaluational tools based on OSCE toward ethical decision making and team communication, 3) investigate the current status of medical students, interns in the opinions of above topics and compare the differences between training and control groups, 4) design the workshop, for teacher training and promote the teaching and evaluational resources.Anticipating results will include: 1) educate more efficiently the medical students and interns the competency of patient-centered care and develop a sense of intimacy and manageable personal responsibility for suffering persons, 2) provide the continuous and efficacious learning evaluation tools, which will promote the quality of teaching and services of end-of-life care.
