2015-02-012024-05-14https://scholars.lib.ntu.edu.tw/handle/123456789/656365摘要:近年來由於自閉症類群障礙(Autism Spectrum Disorder, ASD)的盛行率急遽升高,早期偵測可能罹 病的婴幼兒,將有助於ASD的早期診斷及介入。然而,目前臺灣在幼兒階段ASD的篩檢工具仍然不 足,並且缺乏對於ASD幼兒的早期發展及其核心症狀的了解。因此,本研究的第一階段將中文化在 國外使用良好的幼兒自閉症檢核表-修訂追蹤版(Modified Checklist for Autism in Toddlers, Revised with Follow-up, M-CHAT-R/F),研究將邀請300位具有ASD高風險性、低風險性、以及正常發展的幼兒(年 齡介於16-30個月),由父母填寫M-CHAT-R/F臺灣版,並在幼兒三歲前進行ASD的診斷評估,以分 析此篩檢工具的信效度及準確率。研究的第二階段將邀請30位篩檢為陽性的幼兒,以及另30位性別 配對的正常發展幼兒,在其年齡24、30及36個月時進行兒童發展與行為追蹤,研究將分析高風險 ASD幼兒的早期發展變化,以及與正常發展幼兒的差異。兒童發展檢測將使用標準化的兒童發展評估 工具,評估幼兒在社會溝通、動作、認知及適應行為的發展功能。此外,為了解ASD幼兒在重複與 限制動作的早期核心症狀,研究將使用自動化行為追蹤系統,檢測幼兒在自然遊戲下的行為軌跡,並 分析其靠近人和物體的時間與空間相關參數。研究結果將提供臺灣幼兒可使用的ASD篩檢工具,並 能夠了解和ASD發展有關聯的早期標記,以幫助ASD的早期診斷與介入。<br> Abstract: In recent years, the increasing prevalence rate of Autism Spectrum Disorder (ASD) indicates that earlydetection of infants and toddlers who affected by ASD can help early diagnosis and intervention. However,there is a lack of screening instruments for detecting Taiwanese toddlers with ASD. Furthermore, very fewfollow-up studies to investigate early development of ASD and core symptoms in early childhood. The firstphase of this study is therefore to translate “the Modified Checklist for Autism in Toddlers, Revised withFollow-up (M-CHAT-R/F)” into Mandarin Chinese. Three hundred toddlers and parents that consist of ASDhigh-risk toddlers, ASD low-risk toddlers and typically developing (TD) toddlers (age ranging 16-30 months)will be invited to participate in this study. Parents will then fill out the M-CHAT-R/F Taiwan Version forscreening of ASD. Diagnostic evaluation for ASD will be conducted for all participating toddlers beforeapproaching 3 years of age. Psychometric properties and the accuracy of the M-CHAT-R/F Taiwan Versionfor detecting ASD in Taiwanese toddlers will be examined. The second phase of this study will invite 30screen-positive toddlers and 30 sex-matched TD toddlers to longitudinally follow-up for developmental andbehavior functioning at 24, 30 and 36 months of age. Standardized child developmental assessments will beused to examine multiple domains of child development in social communication, motor, cognition andadaptive behavior. Furthermore, in order to help understand the development of ASD core symptoms inrepetitive and stereotypic movements, toddlers’ behavior trajectory in a free play situation will be trackedautomatically via an automated video tracking system. Time- and space-related variables that indicate child’spreference for exploring people or objects in an environment will be analyzed. The results of this study willprovide a reliable and valid instrument for early detection of ASD in Taiwanese toddlers.
