Cryotherapy is far less effective to treat threshold zone I disease than the more anterior counterpart. Several authors have suggested early treatment for this condition. We use diode laser to treat one extremely premature infant (gestational age 23 weeks, birth weight 650 gm) who had bilateral zone I disease with less than threshold severity. Plus sign was noted in one eye. Under topical anesthesia, photocoagulation delivered through an indirect ophthalmoscope was placed in the widespread vascular area in both eyes in the neonatal intensive care unit. Regression of retinopathy of prematurity with good anatomical outcome was noted after nine month's follow-up.
