2009-08-012024-05-17https://scholars.lib.ntu.edu.tw/handle/123456789/683182摘要:隨著高齡化社會的腳步逼近,整個社會對於醫療服務的質與量,皆有愈來愈高的要求與需求。然而,居高不下的醫療成本,使得醫療服務的創新與醫療資源的運用,成為整個醫療產業提供高品質高效率醫療服務的關鍵。本研究計畫的目的,是針對醫療服務創新的架構與設計,提出最適資源規劃與配置的建議。 本研究計畫將先以「建構以病患為中心的醫療服務」為主原則,探討此一原則的的實質內涵,並應用三大構面的理論深入剖析,包括服務創新、服務品質衡量、與組織績效評估。分析整合此三大構面中的關鍵理論,針對醫療服務產業的運用,整理出此一原則的關鍵績效指標,並以此為基礎發展出醫療服務創新之要素與驅動力。 其次,本研究計畫將深入探討上述要素與驅動力之間的因果互動關係,建構其動態因果關聯網絡。之後運用系統動力學的模型,分析此動態因果關聯網絡,辨識網絡中槓桿效益較高的關鍵要素,並透過模擬分析,探討如何調控這些關鍵因素,進行高效能的資源規劃,進而事半功倍地提升創新醫療服務的品質與效率。 本研究將實際走訪國內某大型醫學中心,深入探討本研究理論模型的實務操作。初步將針對該醫學中心之單一科部或核心流程進行分析研究,隨後再逐步拓展推廣應用至整個醫學中心。 <br> Abstract: As the aging society is approaching, the request for quality improvement and capacity expansion in healthcare service has been growing. However, the soaring healthcare cost makes the healthcare service innovation and its resource planning become the pivotal factor for providing the high quality and high efficiency healthcare service. This research project aims to make a contribution to the task of modeling the dynamic inference network for the factors and drivers to healthcare service innovation, integrating the perspectives of service innovation, service quality, and organizational performance evaluation. This research project centers on the principle of “focusing on value for patients”, discussing its implication and application from three main perspectives: service innovation, quality of service, and organizational performance evaluation. Theories from these areas are to be integrated and applied to identify the key performance indicators for healthcare service innovation, based on which the main factors and drivers for healthcare service innovation are developed. The causal relationships among these main factors and drivers are further investigated to build up the dynamic inference network, based on which System Dynamics (SD) models are developed. Simulation experiments using SD models are carried out to identify the high-leverage elements within the network and to design management policies for effective resource planning and allocation, which hopefully leads to the improvement in the quality and efficiency for healthcare service innovation. To ensure that the theories and models developed in this research project are practical and accepted, close collaboration with a particular main medical center in Taiwan is very essential throughout this research project. In the first stage the main partner is a single department, focusing on investigating its core operations.
