2017-08-012024-05-14https://scholars.lib.ntu.edu.tw/handle/123456789/657511摘要:台灣每年約有一萬七千人會因為腦中風而導致日常生活失能,是成人殘障的第一要因,也是健保資源耗用前三名的疾病。它在急性發病期後,在慢性期病患和家屬要面對的是長期的復健和生活型態的改變和調適,因此中風後的治療與慢性期照護極為重要,也是目前推動長期照護系統時需要整合連結而不可忽視的部分。血壓的控制和部分營養素的攝取(例如: 白蛋白、鉻)曾被研究顯示與中風後的預後相關,但缺之整體性的分析。本研究希望能找出腦中風患者健康促進的因子,提升中風後的預後和生活品質。本研究將使用美國全國健康與營養體檢調查(National Health and Nutrition ExaminationSurvey, NHANES )資料庫及美國國家死亡索引(National Death Index, NDI)資料庫,使用2001年至2010 年間之調查資料,研究母體有52198 人,將樣本分為腦中風患者與非腦中風患者,自變項包括飲食營養成份、運動量、睡眠時數和品質、平均血壓、及各項人口學和身體檢查數據,探討因子間之相關性,使用基因演算法(Genetic Algorithm)搜尋預測中風後長期存活之最佳因子組合及最佳因子離散化切點,以Cox Regression 方法進行存活分析(survival analysis),並建置中風後健康促進之預測模型。本研究結果期望能了解人類日常生活中所攝取各項營養素、運動狀況、睡眠時數和品質、及平均血壓控制值對於腦中風死亡率之影響,以協助醫師指導腦中風患者調整其生活型態,提高中風後的長期存活及長期照護系統的品質。<br> Abstract: Stroke is the third leading cause of death and the most common cause of complex disability inTaiwan. Following the treatment in acute stage of stroke, patients start rehabilitation to improvefunction so that the stroke survivor can become as independent as possible. Literatures demonstratedthe importance of nutrition intake and blood pressure control in patients with stroke, butmultifactorial analysis of health promotion factors in this population is still lacking.Data from the 2001-2010 National Health and Nutrition Examination Survey (NHANES) will beanalyzed to assess the relationship between nutrient intake, amount of physical activity, averagesleep duration and quality, and average blood pressure in adults (n = 52,198). National Death Index(NDI) will provide survival information in patients with stroke. We will develop a prediction modelof long-term outcome in patients with stroke by applying Genetic algorithm hybridized with survivalanalysis to optimize the feature selection and cut-off points for variable discretization.The results of analysis are expected to help the physicians to educate patients how to adjust their lifestyle in terms of exercise and sleep patterns, and dietary intake for improving the long-term outcomeand functional recovery after stroke.基因演算法健康促進預後腦中風genetic algorithmhealth promotionoutcomestrokeUsing Genetic Algorithm to Explore Health Promotion Factors of Survival Prediction in Patients with Stroke
