2015-08-012024-05-14https://scholars.lib.ntu.edu.tw/handle/123456789/658012摘要:本研究團隊已成功地建立4 個量化評估系統,來追蹤頭頸部癌症患者接受放射線治療後肌肉纖維化的嚴重度,包含(1)肌肉橫截面積大小的測量。(2)使用超音波彈性造影進行肌肉縱向硬度(longitudinalstiffness)的測量來代表纖維化的程度。(3)使用超音波彈性造影進行肌肉橫向硬度(transverse stiffness)的測量來代表肌肉的飽滿度(turgidity)。(4)使用超音波的B-mode 影像進行程式化的影像分析,計算纖維組織佔肌肉橫截面的面積比例。延續上一次的研究計畫,本研究計畫欲建立實驗動物模型,以利未來實驗性藥物的投放。第一年研究計畫包含:選擇實驗老鼠的種類,決定放射線劑量,決定追蹤間隔,應用與人類相同的4 個量化指標,取老鼠的肌肉組織切片染色,比較影像學檢查和病理切片的異同。臨床上已廣泛運用肉毒桿菌素來治療頭頸部癌症患者的疼痛,因此,第二年的研究計畫欲檢驗肉毒桿菌素對放射線照射過的肌肉,是否能預防或治療其纖維化,或是僅能緩解肌肉痙攣。本研究計畫讓實驗鼠兩腳皆接受放射線治療,但僅予單腳注射肉毒桿菌素,觀察兩腳的纖維化情形是否有異。本研究第三年的研究計畫,欲尋找已經纖維化的肌肉的新治療方式。預計使用高濃度血小板血漿(PRP),來治療已經有初期萎縮的肌肉,並配合肌力訓練、牽引活動,來促進肌肉的增生及修復。<br> Abstract: Our team had successfully established 4 quantitative evaluation methods to follow up the severity of musclefibrosis in head and neck cancer patients after radiation therapies, including (1) the muscle cross-sectionalarea, (2) the muscle longitudinal stiffness measured by elastography to represent the fibrotic degree, (3) themuscle transverse stiffness measured by elastography to represent the muscle turgidity, and (4) the B-modeimage analysis to calculate fibrotic percentage.To continue, our team wishes to establish an animal model for future drugs studies. In the first year, we aimto determine animal species, radiation dosage, follow up interval, application of the 4 evaluation tools,histological study, and comparison of image and histological reports.Botulinum toxin has been applied to relieve the pain and tightness of head and neck cancer patients afterradiation therapy. Therefore, the research goal of the second year is to study the effect of botulinum toxin onirradiated muscles to see if it can merely relieve a cramping muscle, reverse fibrosis process, or preventfuture fibrotic deposition. The experimental animal would have their both hind legs irradiated but only onehind leg injected with different dosages of botulinum toxin, and regularly followed up for both hind legfibrotic differences.The research goal of the third year is to find out new methods to treat a fibrotic muscle.
