2016-01-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/648406摘要:當前國內的食媒性疾病及相關症狀監測系統,主要是衛生福利部疾病管制署的「法定傳染病監視通報系統」、「症狀監視通報系統」、「即時疫情監測系統 (RODS)」及健保就診資料監測,還有食品藥物管理署的「食品中毒資訊系統」,但針對非屬法定傳染病的食媒性病原感染,並沒有完整建立的實驗室監測。故本計畫目的在於整合疾管署的實驗室監測資料(LARS)與即時疫情監測資料,評估不同監測系統(包括:RODS、LARS 及症狀通報系統)在預警模式的差異與特性,並發展時空分析方法於偵測疫情爆發的時空群聚,同時,也將推估食媒性疾病的發生規模。本研究預計利用三個主要監測資料庫,包括:症狀通報腹瀉群聚、LARS、RODS 等,以及兩個輔助的資料庫,包括:民眾就醫流動資料與通路資料。本研究將進行的資料分析,包括:實驗室資料品質評估、時間異常點分析、時空熱區分析、整合通路與就醫資料的關聯網絡分析、通路關聯分析以及建立病原感染發生率的估計方法。預期結果將進一步瞭解食媒性疾病的實驗室監測系統通報資料品質,建立我國食媒性病原體流行趨勢與流行規模之分析架構;並建構食媒性病原感染時空群聚監測預警與發生率推估方法與分析平台,本研究預計彙整的病原體類型,預期包括;沙門氏菌(Salmonella species)、輪狀病毒(Rotavirus)、諾羅病毒(Norovirus)與A 型肝炎(hepatitis A)等項目。<br> Abstract: Currnet foodborne disease surveillance data in Taiwan is from Notifiable Communicable Diseases Surveillance System, Real-time Outbreak and Disease Surveillance (RODS) of Taiwan Centers of Disease Control (Taiwan CDC) and National Health Insurance Database (NHID). However, there is no well-established laboratory surveillance system for non-notifiable foodborne pathogens. The objectives of the project are to integrate rountine surveillance data from Taiwan CDC’s laboratory pathogen surveillance network (LARS) and RODS and to devlop the early-warning methods for comparing the charactersitics of different surveillance systems for foodborne diseases.We are attecmted to integrate surveillance database from difference sources, including RODS, LARS, Syndromic Surveillance, and other supporting datasets, including patient flow network conducted from NHID and Point-of-Interest (POI) data for food distribution network. The methodology includes the assessment of LARS data quality, development of outlier detection for early warning, space-time clustering of foodborne disease and the procedures for estimating the incidence rate of foodborne diseases. We expect to estiablish the early warning mechanisms for assessing threshold of potnetial epidemic outbreak and estimating annual incidence rate of foodborne diseases.
