2017-05-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/649042摘要:兒童的呼吸道感染性疾病一直是兒童健康的最大議題,因此建立一個全國性跨領域的臨床試驗聯盟,以及完備的呼吸道病毒資料庫乃刻不容緩。因此,由台灣小兒感染症聯盟(TPIDA)建立之台灣小兒感染症臨床試驗合作聯盟(TPDCTC),將可提供為本土重要兒童感染疾病之合作平台。台灣小兒感染症聯盟建立於 2009 年,由台大小兒感染科黃立民教授主持,乃國內醫學中心的小兒感染科共同成立,過去幾年來已有成功執行全國性的兒童肺炎研究之經驗。以TPIDA 為基礎,我們將可以成功收集病人的呼吸道病毒、病人的臨床檢體及其臨床資料,成立呼吸道病毒資料庫,橫向的整合作為將來的臨床研究、基礎研究、和產學合作平台。台灣小兒感染症臨床試驗合作聯盟由臨床出發與學界及實驗室整合,可以更方便提供疫苗,抗病毒藥物治療,與新的檢驗方法之臨床試驗。我們的呼吸道病毒資料庫之特色,除了傳統病毒培養以及聚合酵素鏈鎖反應之外,將納入人類呼吸道上皮細胞培養的技術(HAE),增加敏感性與完備性。對於目前未知的病毒,也可能藉由此技術而有新的發現。過去5 年來本聯盟成功執行許多國際及國內疫苗、治療藥物和診斷試劑的臨床試驗。目前本聯盟的大資料庫和檢體庫亦努力朝精準醫學的領域前進,積極爭取有關兒童感染症分子診斷的臨床試驗。<br> Abstract: Children bear the heaviest burden of infectious illness. An island-wide or even internationalmulti-institution collaboration is especially needed to accomplish any clinical trial in the field ofpediatric infectious diseases. This is the grounds that we established the first Taiwan PediatricInfectious Diseases Alliance (TPIDA) in 2009.TPIDA, led by Professor Li-Min Huang of NationalTaiwan University Hospital, is a collaborative consortium of pediatric institutes in medical centersaround the island. Since 2010, we together have already accomplished many joint researchesregarding pediatric infectious diseases, and more studies about vaccine efficacy and effectivenessare on the way to establish Taiwan Pediatric Diseases Clinical Trial Consortium (TPDCTC). Now inTPIDA, we have about 20 core members, including infectious diseases specialists, virologists,molecular biologists, and more than 30 technicians and assistants working together. We hadestablished a platform helpful for conducting clinical research activities in Taiwan. The databasemanagement platform is already established, and is proved working after a long collaborative studyof pediatric pneumonia. We build up the only respiratory tract virus bank in Taiwan. There aremainly two ways to detect viral pathogens from clinical respiratory tract samples: viral culture inanimal/human cell lines, or PCR. However, there are two major disadvantages. Except for usingconventional virus culture and PCR method, we will integrate the (HAE) system into the collectionof respiratory virus bank. All clinical samples in which no known virus could be detected will becultured in HAE system and stored. Any future translation research activities can utilize the clinicalsamples in the respiratory tract virus bank. Taiwan Clinical Trial Consortium for Pediatric InfectiousDiseases steering meeting is held trimonthly, where updated data on various aspects are generatedand discussed. Our aim is to expand this center from Taiwan Pediatric Infectious Diseases Allianceto an international collaboration. This large data bank and species bank will help this consortium tomove to the new world of precision medicine.
