2013-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/648339摘要:西元 2000 年,美國醫學研究院 (Institute of Medicine, IOM) 出版了兩本具有重大影響力的報告:「人皆犯錯,建立安全的醫療體系」(To Err is Human: Building a Safer Health System)及「跨越品質鴻溝:二十一世紀新醫療體系」(Crossing the Quality Chasm: A New Health Systemfor the 21st Century)。這個報告提醒醫療錯誤導致很多病人受到傷害,醫療體系需要大幅改革,唯有透過強化醫學教育,才有可能以確保病人安全、提升醫療服務品質及應付二十一世紀醫療的需要。而急重症醫療教育訓練更是如此,研究顯示超過一半的美國人,在生命的最後一年住過加護病房,更有約 20% 的美國人在加護病房中結束生命,因此,透過提升急重症醫療的教師培育與訓練課程,培養出良好的急重症醫療教師,藉以提升受訓住院醫師在急重症處理上的各面向能力,包括:急重症醫療知識與技術、臨床倫理爭議及溝通技巧、應對工作時數長與工作壓力大的工作效率。本計畫為三年期整合型計畫—「臨床核心能力培育—急重症醫療訓練課程發展與總體效益評估」中的第一子計畫—「急重症醫學教師培育與訓練計畫」,預計將達成下列目標:第一、探討現今住院醫師「急重症醫療」教育訓練課程的教學內容第二、依據學習理論設計針對資深住院醫師「急重症醫療」的教師培育與訓練學習課程第三、檢驗以資深住院醫師擔任「急重症醫療」教師的教學成效<br> Abstract: In 2000, the Institute of Medicine (IOM) in the United States published two influencing booksentitled “To Err is Human: Building a Safer Health System” and “Crossing the Quality Chasm: ANew Health System for the 21st Century.” Both books reminded that medical errors usually harmmany patients, and the health care system needs to be redesigned. By reinforcing medical education,patient safety will be guaranteed, and quality of medical services will be promoted in face of themedical needs in the 21st century. Critical care medicine is particularly important according to thetwo books. Some studies in the U.S. showed that more than 50% of patients had been admitted to anintensive care unit during the last year of their life. They also reported that 20% of the U.S. patientsdied in an intensive care unit. As such, by promoting the teaching skills of educators in critical caremedicine is extremely important. By doing so will the capabilities of residents in critical caremedicine will be promoted, including the clinical knowledge and clinical skills, resolving clinicalethical conflicts and communication skills, and adjusting workload.This research proposal (Faculty Development for Critical Care Medicine: Residents asEducators) is one of the four sub-proposals included in an integrated proposal (Cultivating ClinicalCompetence in Critical Care Medicine—The Development and Appraisal of a ComprehensiveEducation Program for Residents) organized by Professor Ni, the Chairman of Department ofMedical Education, National Taiwan University Hospital.
