2016-08-012024-05-17https://scholars.lib.ntu.edu.tw/handle/123456789/672781摘要:心臟疾病是國人的第二大死因,而院外心跳停止(OHCA)也是重要的公共衛生課題。要提高存活,須優化現有生命之鏈(chain of survival),從緊急救護網,向社區推進,一方面提高民眾意願執行心肺復甦,並在公共場所部署自動電擊器(AED),以便最早期間進行電擊。「優化生命之鏈整合型計畫:從緊急救護體系前進社區」之整體目標是透過(1)成本效益評估,(2)平面與立體佈建,以及(3)創新民眾教育等三構面,全面優化社區生命之鏈。本計畫為其子計畫一,針對近年來公共場所電擊器佈建與簡化民眾心肺復甦教育兩大策略,進行成本效益評估。本子計畫將以三年時間,達到下列目的:1). 跨區域心跳停止登錄體系中長期存活預後與生活品質的定義2). 台灣地區OHCA 存活狀態以及預期餘命的估計3). 測量台灣地區OHCA 存活者的生活品質4). 台灣地區治療OHCA 短期成本和長期成本的估計5). 評估台灣地區公眾場所AED 佈建和簡化民眾CPR 訓練計畫的邊際成本效益本計劃的實施,不但對公共場所AED 佈建以及簡化民眾CPR 教育兩大策略,提供中長期存活,生活品質,以及成本效益之重要本土科學證據,也是優化生命之鏈整體策略的重要拼圖。<br> Abstract: Cardiac diseases are the second leading cause of death in Taiwan. As many cardiac deathspresent as out-of-hospital cardiac arrests (OHCA), OHCAs carry significant medical and publichealth priority in the community. To improve patient survival, it becomes imperative to advancecurrent chain-of-survival further into the community by promoting community bystandercardiopulmonary resuscitation (CPR) and public access defibrillation.The overall objectives of the integrated project:“Optimizing Community Chain-of-Survival:From EMS to Community”, is to strengthen community Chain-of-Survival through threeapproaches: 1) cost-effectiveness evaluation; 2)device deployment strategy, and 3)innovations incitizen CPR education. In this first arm of the integrated project, we will conductcost-effectiveness evaluation on the two major community Chain-of-Survival optimizingstrategies, including public access defibrillation (PAD), and simplified citizen CPR education.In this three-year project, we will achieve the following objectives:1). Define long term survival and functional outcomes measures for OHCA registry in Taiwan.2). Assess longer term survival status of OHCAs in Taiwan3). Measure quality of life among OHCA survivors in Taiwan4). Assess short-term an long-term treatment costs of OHCAs in Taiwan5).
