2009-01-012024-05-17https://scholars.lib.ntu.edu.tw/handle/123456789/684420摘要:我國農民健康保險(以下簡稱農保)正式實施於1989年,原為維護農民健康、增進農民福利所辦理的社會保險制度,惟自1995年全民健康保險開辦後,原來的醫療給付,已從農保移至全民健保體系中執行,目前僅提供農民生育給付、殘廢給付與喪葬津貼等三種保障。然而,農保推行迄今將近20&#63886;,雖&#63884;經多次修法改革,但制度定位與內涵的結構性問題,仍舊存在。舉其要者,含農保監督、管理與執行體系的繁複;農民投保資格的分歧和認定困難;無最高投保&#63886;齡之限制;保險費率釐定的不合理,以及政府財政挹注過高等法令制度之寬鬆設計,均是造成農保整體結構日趨不良的主因,進而導致農保財務的嚴重虧損。值得注意的是,原為暫時填補老年農民經濟安全缺口的「老年農民福利津貼暫行條例」,也因為老年農民福利津貼的多次調高給付額度,反而演變成農保體質逆選擇與加速惡化的道德風險來源。基此,本計畫的主要研究目標有兩:(1)檢討農保與其他社會保險制度的公平性問題,並擬定農民健康保險制度之改進對策。(2)檢視我國農民福利保障的現況與制度缺失,規劃未來農民福利的發展方案,完成農民福利規劃報告。<br> Abstract: The Act of Farmers’ Health Insurance (FHI) came in force in 1989, which aimed at ensuring farmers’ health condition originally. Since the implementation of the National Health Insurance (NHI) in 1995, all medical benefits of FHI were taken over by NHI. The remaining social benefits provided by FHI embrace only the maternity benefit, disability benefits, and funeral allowance. Although the FHI has experienced numerous modifications and reforms, its institutional problems are still unsolved. The main defects of the Farmers’ Insurance system caused a long-term deficit of FHI’s finance, which includes: no upper limit of insured’s age, difficult to qualify the eligible or to identify the working farmers, and too low insurance premium rate. Besides, the “Old-age Farmers’ Welfare Allowance Act, OAFWA” is a temporary regulation which provides quasi-pension to the elder farmers who are over 65 and with 6 months insured period. The additional old-age welfare allowance results also in adverse selection and moral hazard of FHI. There are two main proposes of this project: firstly, to explore the inherent problems and defects of Farmers’ Health Insurance and a reform proposal should be suggested. Secondly, to evaluate the welfare status of farmers and develop a comprehensive social security net program.農保制度改進對策及農民福利規劃計畫
