2017-08-012024-05-18https://scholars.lib.ntu.edu.tw/handle/123456789/704442摘要：近年來，基於病患安全之考量，醫學教育的成效已慢慢傾向利用標準化評估工具，如客觀臨 床能力試驗評估(OSCE)、操作技能直接觀察評估(DOPS)、臨床技能評估(Mini-CEX)等，來 進行“勝任”（competency)與否之評估。美國急診醫學會已將急重症超音波列入急診住院醫師訓練之必要課程之一，並且建立11 項核心應用指標。而台灣至今亦無標準化工具來評估急診超音波之訓練。本研究目的即是利用美國急診醫學會已建立之急診超音波的核心應用指標，建立標準化 評估模組。研究對象為台大醫院急診部之資淺醫師，包含畢業後一般醫學訓練醫師(PGY)及急 診部之住院醫師，預計100人。研究時程預估為期兩年，第一年為建立急診超音波之評估模 組，第二年將進行評估模組實作及演練，進而評估該模組之合適性及建立其信度及效度。本研究之重要性在於改善傳統醫學教育之窠臼，建立標準化評估模式，評估資淺醫師在 急診超音波操作上之勝任程度；建立本土客觀評估系統，建構以住院醫師畢業能力為考量之 評估模組，未來亦可用於急診專科醫師認證，提升急診醫療品質；且由於目前超音波評估模 組之研究鮮少且莫衷一是，本研究可建立本土化評估系統，促進未來與各傑出醫療機構合作之契機。<br> Abstract: The assessment method, including objective structured clinical examination (OSCE)、direct observation of procedural skills (DOPS), and mini-clinical evaluation exercise (Mini-CEX), were used for evaluation for physician competency in recent years. The American College of Emergency Physicians established 11 core applications of emergency ultrasound in 2008. However, there is no standardized assessment tools for emergency ultrasound (EUS) to evaluate emergency physicians’ ability in Taiwan.The purpose of this study is to establish EUS competency assessment modules using the core applications. The junior physicians of the emergency department at the National Taiwan University Hospital will be enrolled. The study period will last for 2 years. Establishing the EUS competency assessment modules and items will be done in the 1st year and constructing fair validity and reliability of this modules will be conducted in the 2nd year. The results will be reviewed and could be filed as a reference for the following EUS teaching program.This pioneer study can provide experience of EUS competency assessment and contribute to current medical education. Moreover, it can establish unique EUS competency assessment modules for the accreditation of emergency medicine to improve quality of care. Furthermore, the assessment modules may be used in other countries to improve international cooperation in the future.
