2018-08-012024-05-18https://scholars.lib.ntu.edu.tw/handle/123456789/706750摘要:兒童過重及肥胖之情形過去三十年來不分國內外均日趨嚴重,台灣學齡期兒童過重及肥胖的盛行率也高達31.8%,兒童過重及肥胖不僅與成年時期的過重和肥胖形成有關,也增加罹患各種急慢性疾病和早期死亡的風險。睡眠問題已被證實與過重及肥胖的形成有密切關係,而學齡期是教育孩童良好睡眠習慣及健康體重維持的最佳時期,但全球目前還沒有任何學齡期兒童過重及肥胖介入措施納入睡眠健康之概念並利用手機軟體作互動及給予孩童即時回饋。本三年期研究計畫之目的為發展及測試FAMILY兒童生活型態介入措施方案(Family-based And Mobile-assisted Intervention for Lifestyle behaviors in Youth, FAMILY),研究目標為: 1) 開發及前驅測試FAMILY手機軟體用於記錄及互動介入學齡期兒童睡眠、活動、和飲食行為之可行性和可接受性及可用性,2) 利用隨機分派之實驗設計,評估FAMILY方案對改善學齡期兒童睡眠及降低兒童過重及肥胖之成效。本研究團隊預期透過行動健康照護之理念並融合現今手機科技,能以更有效且更有趣的方式促進學齡期兒童健康睡眠及體重之維持。<br> Abstract: Childhood overweight and obesity is a major global health concern and is reported in 31.8% in our nation's school-age children. Childhood overweight and obesity is not only associated with overweight or obese status in adulthood, but also with an increased risk of morbidity and premature mortality. Sleep and weight are intricately related with a recent meta-analysis including 56,584 children showing that each 1 hour/day increment in sleep was associated with a 21% reduction in the risk of overweight and obesity. Although the elementary years are an optimal time for understanding that children's sleep needs are essential to maintain a healthy weight, no intervention studies conducted in Taiwan or elsewhere in the world for childhood overweight and obesity have applied mobile technology and incorporated sleep health with screening for potential sleep problems for school-age children.The purpose of the three-year study is to develop and evaluate a FAMILY program (Family-based And Mobile-assisted Intervention for Lifestyle behaviors in Youth, FAMILY) for improving sleep and lowering risk of childhood overweight and obesity. The specific aims of the study are to: 1) develop and test the feasibility, acceptability, and usefulness ofthe FAMILY mobile application for school-age children and their families to document sleep, activity, and eating behaviors and to receive interactive individualized health advice, and 2) implement an open-label randomized controlled trial to evaluate the effectiveness of the FAMILY program to improve sleep and reduce adiposity in school-age children. Healthy and normally developed children aged 6-8 years without any acute or chronic illnesses will be recruited to participate in the study.The FAMILY program is underpinned by behavior change theories combined with mobile health support to increase parent-child awareness and provide practical solutions about childhood sleep and weight problems. The program has an initial intensive phase consisting of 12 weekly sessions (0 to 3 months) and a maintenance phase consisting of another 3 monthly sessions (4 to 6 months). Data collection includes baseline (T0) and 3 (T1), 6 (T2), and 12 months (T3) post-randomization. We proposed a family-based intervention because families constitute the primary environment for the young and school-age children typically point to families as useful in providing guidance for improving sleep, activity, and eating behavior. We use a mobile application as an innovative intervention modality which provides a concrete assessment and behavior change monitoring tool. Children and families can choose a pet to raise from our mobile application and the pet will mimic the child's lifestyle behavior and body weight status. When the child's behavior meets the task goals, the pet can evolve and level up into a new form which can serve as a direct feedback and motivation for the child.
