2015-08-012024-05-18https://scholars.lib.ntu.edu.tw/handle/123456789/711388摘要:膀胱過動症及尿失禁是婦女常見的下泌尿道症狀,不僅造成生活上的困擾也影響生活品質,然而國內外懷孕至產後期的世代研究報告仍然相當欠缺。本研究計畫目的是描述及比較未懷孕階段、懷孕產後階段婦女膀胱過動症與尿失禁的變化及差異,檢視懷孕、生產及相關因素對兩者的影響,及其對婦女健康造成的衝擊,以及確認懷孕期至產後期所有調查時間點所形成的軌跡及此軌跡的影響因素。研究方法採取世代研究設計,長期追蹤婦女懷孕期至產後期的膀胱過動症。研究對象為某醫學中心產檢及生產的婦女,預計納入約400 位符合條件的懷孕初期個案進入研究。每位參與者預計完成人口學、懷孕、生產等個人資料表;尿失禁、膀胱過動症、睡眠品質及憂鬱量表。預計3 年完成本計畫,長期追蹤的時間點包括懷孕初期、中期、後期,產後3-5 天、1 個月、3 個月、6 個月及1 年。統計分析包括ANOVA, ANCOVA, GEE regression, mixed model, and trajectoryanalysis。預期研究結果將能深入了解婦女在母育階段,下泌尿道症狀多面性與縱貫性現象,建構周產期婦女性健康理論模式,提供健康專業人員對下泌尿道功能障礙處置之參考。本研究計畫將延續計畫主持人過去在母育階段婦女健康議題的研究成果,朝向提升周產期照護精緻化的目標邁進。<br> Abstract: Overactive bladder and urinary incontinence are the common symptoms of lower urinary tractfor women, which may influence their quality of life. However, few reports have addressedoveractive bladder and urinary incontinence across pregnancy and postpartum by validatedmeasures and cohort design. The specific aims are to (1) describe and compare the differencesin overactive bladder and urinary incontinence during pre-pregnancy, pregnancy andpostpartum; (2) identify factors influencing overactive bladder or urinary incontinence atdifferent periods, before, during and after pregnancy; (3) examine the impact of overactivebladder and urinary incontinence on women’s health ; (4) identify the trajectory of overactivebladder or urinary incontinence and the factors that influence this trajectory throughpregnancy and postpartum. A prospective longitudinal investigation will be performed inwomen at pregnancy and postpartum. All participants will complete questionnaires that assessdemographic characteristics, obstetric and birth history, female overactive bladder, quality oflife, sleep quality and depression. Participants who are in early, middle, late pregnancy, 3-5days, 1st, 3rd, and 6th month and 1 year postpartum will be contacted to complete thequestionnaires by mail or at study site clinics. The sample size was estimated by drop ratesand power calculation. Approximately 400 women in early pregnancy will be recruited at amedical center in northern Taiwan. The study will be conducted over a 3-year period. Thecharacteristics and trajectory of overactive bladder and urinary incontinence in the studypopulation will be examined, and a theoretic model during pregnancy and postpartum will beidentified.
