Inthe United States, there are with approximately 140,000 new cases and 50,000deaths per year. The incidence of colorectal cancer has increased in Taiwanrecently, with over 10,000 new cases and 4,000 mortalities per year. Despite theemergence of new biologic agents, availability of several new chemotherapeuticdrugs, and use of various combinations thereof, none of these treatmentmodalities is curative for patients with advanced colorectal cancer. This problemraises questions as to whether current systemic anti-colorectal cancer treatmentsare targeting molecular mechanisms or tumor cell populations that are trulycritical to cancer growth.It is gradually accepted that only a small fraction of cancer cells with theproperty of stem cells, known as cancer-initiating cells (CICs) , are responsiblefor tumor growth and metastasis. However, in colorectal cancers, markers ofcancer-initiating cells and its relationship with clinical outcomes are not clearlyunderstood. Our previous results showed the intensity of CD133, which is aproposed marker of CICs, immunohistochemistry (IHC) staining on the primarycolorectal tumor is associated with patients’ prognosis. But positive staining rateis not high enough. Also the mechanism by which these cells can inducemetastasis or recurrence is not so clear at present. In this project, we will try toinvestigate the role of cancer-initiating cells in colorectal cancer progression andmetastasis and achieve three aims, including 1. establish the connection betweenthe CICs and cancer metastasis or recurrence by IHC staining of candidate CICsurface markers on surgical specimens, especially focusing on peritonealcarcinomatosis in colorectal cancer patients; 2. find the mechanisms ofperitoneal carcinomatosis in terms of cancer-initiating cells and 3. screencandidate compounds and evaluate their therapeutic effects against colorectalcancer carcinomatosis.
